National Guard at the entrance asked for ID and proof of vax. I'd lost my CDC card, and they didn't accept my Excelsior Pass Plus, but they did accept my printout from the Connecticut immunization registry (was originally vaccinated there) and let me in. I'd also brought a printout from the NYC immunization registry and my CDC VAMS certificate just in case, but the CT one worked. They really seemed to expect the original CDC card, though.
A second guard escorted me from the entrance to the registration room. A third guard registered me and then I was sent to a different room to have the booster administered. The person who gave me the shot (not a guard) used the info from my Excelsior Pass Plus to create a new CDC card that included the info for my two Pfizer shots as well as the booster. Very happy to have a new card!
I then went to an observation room which was monitored by a guard. After 15 minutes I was dismissed, and I exited without an escort.
Aside from the odd fact of the Excelsior Pass being good enough to create a new card from, but not good enough for admittance, it was a smooth experience. Nobody asked for proof of eligibility, but I'm guessing that's because they care more about just getting people the shot. They also didn't ask for the CDMS registration ticket that I got when I made the appointment, insurance info, or proof that I completed the NYS vaccine form.

I know someone asked this more generally a while back but I was wondering about Medgar Evers specifically, as thatβs where Iβve scheduled my appointment.
Iβm very concerned about fainting/triggering the vasovagal reflex because my mother fainted when she got hers and I nearly fainted just watching my SO get his. So it would be helpful to get a sense of the kind of accommodations Medgar has.

My appointment was last night, Thursday March 4th in the evening. Make note of the address on your confirmation ticket from the NYS DOH.
The address is: 231 Crown Street.
NOT 1650 Bedford Avenue which is what the address is if you Google Map Medgar Evers College.
I got to the site early in case there was a line or forms to fill out.
You enter in through a tent and are asked to have these items ready:
There was virtually no wait. FEMA staff directed me to have my items ready. I had a passport for ID, they said they were being funny about passports. I explained I had my utility bills, they said that was fine. FEMA directed me towards the entrance of the building. From there, staff from the Air Force organized the line. I was told again to keep my items ready.
I was then directed to a table in the lobby area where Air Force staff had my information on their laptops. To verify my address, I showed them my Con Ed and National Grid bills. After they confirmed I said who I said I was, they directed me to another line that led into the main gym area where the majority of the vaccine pods are located. We were told to take our coats off and be ready to be vaccinated. Before entering the gym, a young woman directed individuals into different pods. I was told to follow a young man who hand carried the vaccine to a pod where they asked me a few questions: did I have any allergies to vaccines, did I have any questions.
At no time did they ask what my comorbidities were nor did they ask me for proof.
They gave me my shot and as they gave me the shot, they also made my appointment for me. I was given my appointment card as well as a sticky note indicating how long I should hang out in the waiting area. There was another Air Force staff member by the exit and after I waited the 15 minutes I left after he verified I had stayed the required length of time.
The entire experience lasted exactly 27 minutes. Overall, everyone was professional, friendly and efficient. 10/10 would have FEMA and the Air Force handle pandemic vaccine logistics again.

Evers was assassinated in 1963 by Byron De La Beckwith, a member of the White Citizens' Council in Jackson, Mississippi. I got the Pfizer vaccine at Medgar Evers College in Brooklyn this morning after making an appointment over the weekend. This site is currently open to residents of select Brooklyn zip codes (11207, 11212, 11208, 11206, 11233, 11213, 11221, 11226, 11236, 11216, 11225, 11210, 11203, 11238) with 20K+ appointments still open beginning in early March.
I took a Lyft there and someone saw me getting out of the car looking confused and pointed me in the direction of the line. It was quite long (wrapped around the block) and I was worried I'd be waiting all day, but it moved quickly and it ended up taking about 15 minutes for me to get to the front. My appointments was for 9:30, I got to the site around 9:10, and I got the vaccine around 9:40.
Next to the main line, there was a sign-up line at the site for people who had come seeking vaccination but hadn't signed up for an appointment; volunteers (I think?) helped them sign up for a future appointment but they weren't able to get the vaccine today. The people in this line mostly seemed to be older people who may not have internet access or be comfortable navigating the online sign-up.
Once I got inside the building, I was directed to a check-in desk and asked to confirm my name, address, and phone number and show proof of residence (I brought a copy of my lease since my driver's license has an old address on it). I wasn't asked for documentation of my health condition or to fill out any further forms, since I'd already filled out everything online.
I was then directed to walk to a room full of vaccination stations (basically a desk + chair partially enclosed by a curtain with 2 nurses inside) and I stood in line for another couple minutes until one was open. The nurse at the station briefly confirmed with me that I'd read the provided information on the vaccine when I'd signed up and that I wanted to get it, then the other nurse gave me the shot. The first nurse gave me a vaccine card with the date for my next dose. She told me I should expect to get a follow-up email later this week to select a time for that appointment.
After that, I was directed to a waiting area full of plastic seats to wait for 15 minutes. When those 15 minutes had passed I left, showing a person at the door my vaccine card which confirmed the time I'd gotten the vaccine and that it had been 15 minutes.
The whole process, from getting in line to leaving the building after the waiting period, took less than an hour.

Before:
I got the appointment via https://am-i-eligible.covid19vaccine.health.ny.gov/. I gave my insurance information when signing up, and filled out the New York State COVID-19 Vaccine Form from the link in my confirmation email.
I don't have a NY license, so I found a copy of my lease in my email to show proof of residency (and brought my out-of-state driver's license).

The entrance is at 231 Crown Street, and when I walked up there was a line of maybe 10 people there. There are both volunteers and members of the National Guard and Air Force, volunteers in reflective vests and servicemembers in their fatigues.
I had to show proof of residency to a volunteer, then walk a few yards down the line and show proof of residency to a National Guard servicemember. He directed me into the actual building where another servicemember took my temperature (pointed a thermometer at my forehead). Once that looked good, he directed me to a numbered check in station.
A third servicemember looked at my ID and proof of residency and asked for my birthday. She matched my info with the info in their system and showed me where to walk to next.
There was a short line waiting to go into the gym where the vaccines are administered. A servicemember told me which booth to go to (they're numbered) and I walked to it and sat down.
In the booth, there were two servicemembers -- one to check if I'd done the paperwork and consented to getting the Pfizer vaccine, and one to administer the vaccine. She asked which arm I wanted it in and wrote down my name and birthdate on the vaccine record card, then had me review it for accuracy.
Once the shot was done (super quick, barely felt it!) I was instructed to sit in a chair in the center of the room. I got a sticker with the time I had to wait until on it.
They had some balloons showing that the site has administered 125,000 doses thus far and a ton of signage telling you that the public doesn't have permission to take photo/video inside.
Once the time on my sticker came, I walked over to the exit where a volunteer was checking cards and the time you were allowed to leave.
I'd estimate it took ~5 minutes from walking up to the first line to getting the shot in my arm, then another 15 minutes to wait for side effects.

Went to 231 Crown St, arrived roughly 10 mins before my appointment time. Hovered for ~10s in front of the long white tented hallway before a Natβl Guardsman asked for ID (I have a NY license w a BK address), confirmed I was a Brooklyn resident. Several ID checks at the door - one including an appointment confirmation with a guard on the computer (asked for ID and confirmed phone number and DOB). Led down into a gym into a cubicle, was asked several questions that mirrored the pre-form questionnaire. Was asked βDo you consent to the Pfizer vaccine?β in different ways, several times. You are then sent to sit in one of the open chairs in the middle of the gym until your handwritten time to leave after 15min (checked at the door). Within 5 minutes of my arriving, I had gotten my shot.
Second shot scheduled according to your real-time first shot. β21 days from today, around this same time. Bring your appointment card. Youβll get an email in the next few days - but even if you donβt, show up with your appointment card and youβll get your second shot.β
I was never asked about eligibility or asked to provide proof of. I was also not asked to show my appointment confirmation or if I had filled out the pre-appointment form. These must be uploaded in the system somehow.
The environment was calm, organized, and uncomplicated. Thank you to the Medgar Evers campus, staff, and servicemen and women for your contribution to the cure.
UPDATED, 4/2/21 second shot: Same experience as first. Questions on this second round included a rerun of the questions prior to my first shot. THEY ASKED ABOUT ELIGIBILITY, but only required a verbal response. No official forms or physical proof was required.

The event was very peaceful, with no mention of violence or arrests made reported by the New York Times. Despite the show of force, U.S. nuclear weapons programs were not limited until after the end of the Cold War, several years later.

Had an appointment at 1:10. I was a little earlier (10 minutes or so) but no one said anything about. You could also register to get the vaccine there for another date) You wait in a line and they check your registration and your ID twice (once at the door and another at a table.) They direct you to a table where someone asks you if youβre feeling well, if youβve had other vaccines, etc. They give you the vaccine (painless, took a second). You get a card with a second appointment (I think I book that myself and they give you a date to get it- Iβll wait to see if I get an email about it). No one asked about eligibility but Iβm also in one of the listed zipcodes and I think they just want as many people vaccinated as possible.
