First post here but I have lurked around and appreciated the insight and support. I am an ortho-spine from a non-US developing nation. Finished fellowship in mid 2020 [ I know..] and currently on the verge of a break-down frustration I hope to vent productively here.
So I am back to the hometown and in solo private practice - no other option of a salaried job where I am. Practice runs on having a PRO who goes around offering 'cuts' to referee GPs/other consultants/guy who dolls out govt benefit cards/ward apprentices in other hospitals--the list is long. I have a massive moral inhibition against the practice and that, combined with the saturation of specialists, have made my practice an extremely low volume one.
I was trained well and came out with confidence doing the bread and butter stuff - discs, lamis, cervical and thoracic stabilisations etc . But I have operated so less - just 10 cases in entire 2021 -- that my confidence to do even basic stuff is shaken to the core. I operated one case in last 4 months and I am at a loss about what should I do. I am wrecked by anxiety about a potential case that comes because I operate so less. I constantly think about where should I get cases, how will i do them IF i get them and so on.. Whatever few cases I have done, I have done quite well but the anxiety is delaminating my existence one bite at a time.
I am trying to remedy this by offering free services to the local govt hospital but the local politics is not allowing me to do so. Few peripheral places I go to have scanty cases and while I hope this year might improve with an attachment to a not for profit place that is coming up -- I am seriously worried my confidence and my entire personality is getting deconstructed by my ongoing experience.
Has anyone dealt with such low volume issues before? How do you deal with the anxiety? While money is not an issue as I come from a well to do family, my self confidence is shattered and I am worried about my future as a surgeon if this goes on in this early phase of career.

Hey, friends! My art group is putting together a live performance of Erik Satie's Vexations starting this Friday, August 13th at 8AM and going through August 14th at 8PM (36 hours).
โIn order to play the theme 840 times in succession, it would be advisable to prepare oneself beforehand, and in the deepest silence, by serious immobilities.โ โ Erik Satie
Prior to 1963, Erik Satieโs Vexations was a relatively obscure piece of 19th-century piano music. But John Cage reimagined this work and its possibilities when he chose to take Satieโs instructions literally. His original 18-hour performance became a cultural touchstone for pianists, inspiring performances of greater and greater duration. The longest recorded concert lasted 35 hours in Paris in 2012.
In a unique collaboration between sound and movement, Interdisciplinary Arts Collective musician Aaron D. Smith will be performing Vexations alongside 6 dancers in a 36-hour concert, accompanied by visual art by Dani Mendez. Beginning the morning of Friday, August 13th, and continuing non-stop until the evening of August 14th, the performance is open for audience members to come and go as they please. We invite listeners to join us for 10 minutes, an hour, or 36 as we explore the mutability of time, the explosive potential of stillness, and the consequences of sustained effort alone and in partnership.
