I had a rheumatologist for about 8 years then he retired and I really haven't liked his replacement at all. Besides fibromyalgia, I was recently diagnosed with RA and celiacs by her. To try to shorten up the story her reaction to everything is to farm me off to her recommendations of a gastroenterologist and a pain management person. My old rheumatologist put me on triamterene and now she refuses to refill the prescription anymore after doing so for about a year saying I will need to go to my regular doctor to get it refilled. She and I have had some disagreements recently so I can't help but feel this is in retaliation for me sticking up for myself. It's really hard on me to have to drag my ass off to different appointments. Has anyone else had triamterene prescribed by their rheumatologist or am I being too sensitive?
Oh my god now I remember why I take it only when I need it. I’m peeing every 30-40 minutes or so. It’s torture. I’ve gotten dehydration headaches. Help me. 😖😂 (vent over)
I was just diagnosed and prescribed triamterene. I have concerns about side effects (low blood pressure, fainting/lightheadedness, kidney problems, dehydration, gastric issues, etc). I wanted to know if anyone here has taken this medication and what their experience was. Of course everyone is different, I’m just trying to gauge what side effects seem more common. Also, if you were on this medication, did it make a difference in your symptoms?
For those that are on TRIAMTERENE-HCTZ 37.5, I’d love to know what dose you settled on. Im still trying to find what works for me, as I just relapsed after a few good months, and steroid injections don’t seem to be working either. My doc is going to take me up in dose to see if that works. I’m mostly just curious where my dose falls on the spectrum. How much are you all taking? Right now I’m at 25mg.
Also has anyone found that a combo of this and another intervention was what worked for you? I don’t have severe attacks: just slight dizziness, tinnitus, fullness, fatigue, and brain fog that last for months at a time.

My new otologist switched me off Triamterene-HCTZ and onto just HCTZ with a Potassium supplement. His opinion is that HCTZ + Potassium gives better results than Triamterene-HCTZ. Just wondering if anyone has taken or is currently taking this combo and if so, what are your experiences? Thanks
I recently came across this combination for a 90 year old female, who was on triamterene/HCTZ for CHF and was prescribed Bactrim for a UTI.
My first thought was hyperkalemia because triametrene is a K sparing diuretic and Bactrim acts at the distal convoluted tubule promoting K retention.
I ask the pharmacotherapy experts about this combo because my preceptor didn't think anything of it and wanted to just bounce, but my immediate reaction was to want a 12 lead ECG (looking for peaked T waves and a wide QRS with calcium chloride at the ready) and draw labs for evaluation upon arrival at the ER, in addition to running in a fluid bolus of normal saline.
Hyperkalemia with Bactrim is a real thing and coupled with a K sparing diuretic I feel like its negligent not to investigate the K as well as what the myocardium is doing. Thoughts?
I recently had a 90 year old patient call 911 who was on the above combination, where triamterene is a K-sparing diuretic and Bactrim is also know to act at the distal convoluted tubule promoting K+ retention. So essentially a double whammy. Upon our arrival she was extremely weak, but also had been experiencing vaginal bleeding of an unknown amount.
My medic preceptor wanted to BLS her and jet to the hospital, but with the above combo I strongly suspected the possibility of hyperkalemia and wanted a 12 lead ECG, as well as starting a line and drawing labs. We kind of butted heads about elevating the call to ALS, including giving her a fluid bolus. Meaning my preceptor just wanted to load her and give her a ride to the ER and that's it.
My wife who is a clinical pharmacist and was on shift at the hospital we transported to absolutely cringed at that combination in a 90 year old patient, and said it was entirely appropriate to suspect hyperkalemia in a patient with unknown renal function. Cases like this is where my doctoral and masters degree knowledge seems to butt up against my current EMS training. I had red flags going off in my head about that combo and checking a 12 lead as well as drawing labs for Stat evaluation upon arrival I felt was entirely reasonable. Plus with unknown bleeding amounts, hemoglobin and Hct would also need to be evaluated as well. The current protocols for transfer is if labs are drawn by EMS, and done properly, they are sent immediately off for evaluation as in taken right away by the tech form the ER to the lab.
In the end her K was 5 but her Hgb and Hct were in the toilet and she needed a transfusion, but I could not have known that a priori. My question to you on the other side of the EMS transfer is was my clinical judgement reasonable?
Spent hours reading papers and my textbook and can't find a SINGLE thing that suggests a mechanism for triamterene (C12H11N7)'s dissolution in the stomach/in water. It's poorly soluble in water and I need to figure out the equation to prove that it mostly stays solid for a personal project.
My ENT prescribed me Triamterene 37.5mg as a diuretic water pill for meniere's and according to Drugs.com the side effects read like War & Peace. I know they are supposed to be rare, but it's kind of ridiculous that a water pill would supposedly have so many side effects
Are any of you currently on a water pill? If so, which one? Any one had tried eating more foods that are natural diuretics? How did it or did it not help?
