Received my diagnosis last week and meeting with my primary care doctor to further discuss results and treatment this week. I have a great relationship with him but I know he has very little experience with sleep disorders and IH specifically. I tend to be sensitive to meds and would prefer to work with someone that specializes in this area. Do I start with looking for a psychiatrist, PCP, sleep specialist, etc?
Bonus points if you know someone excellent in the Midwest US!

Personally I'd prefer to just skip the middle man (no offense to any primary care physicians out there). But Idk if this is something that will be an issue when I try to schedule an appointment.
It's outside business hours for the place I'm trying to get an appointment at. Just curious now.
Note: I still get regular required checkups through my job (they have a physician for that purpose) so it's not like I'm totally neglecting regular healthcare.

That all an FM doc is good for is following JNC hypertension guidelines. But even that is questionable, since FM docs don't have the same training as a cardiologist.
(Something I am noticing more and more often in a physician advocacy group).

I work for an SCI organization and I'm working on an info resource (in part) about communicating with non-SCI-specific physicians (for example, primary care, pain management, ER docs etc) about your needs/care. How do you approach sharing your knowledge or research evidence with your physician about SCI? How do you deal with communicating your needs to a physician you probably won't see again (ie an ER doc or Locum)? Any tips you'd offer for people with recent injuries on working with their doctors? What has worked for you? What have you learned about this from experiences you've had?
I welcome input from anyone with SCI (or family members), but I especially would love to hear tips from Canadians with SCI, and ambulatory folks with SCI.

Saw a few orthopedic specialists in my area for some tendon issues, here's how it went:
I wore a small brace for a week or two and it also just made it worse for my wrists. Looked up studies online and bracing makes fcu issues *worse* since the wrist just isometrically contracts against the cast's resistance.
Why do these doctors give such bad advice? All anecdotes, research trials, etc. show that casting for tendon issues after initial onset isn't great, and I had horrible experience with it. Do they just not know? If so, what do they do in school for so long? Do they just not give a shit?

How accurate is this?
31 year old, 5β7 male, approx 165 lbs
So for about a week now I have been waking up with a dull pain in my upper back. Feels like it is in the center of my back in between my shoulder blades, its a dull pain that feels worse when I flex my shoulder blades (sticking my chest out).
Its not a constant pain as it seems to go away throughout the day, but its getting to the point where its affecting my sleep as if I wake up in the middle of the night, i cant fall back asleep because of the pain.
At first I thought it was due to sleeping on my side so I tried changing positions and sleeping on my back, and also tried sleeping on a different bed in the house but still experiencing the pain. I cannot think of any physical strain that wouldβve brought the pain on.
Basically I wasnt sure the best course to get this checked out and who to see. Whether i should make an appointment with my general physician, walk into an urgent care, or go right to a specialist. I was leaning towards an urgent care as id like to be checked as quickly as possible since this is becoming a real nuisance, but i wasnt sure if theyd be best equipped to get to bottom of the problem.
My doctors who brought me into this world retired and all of my medical records and history and knowledge went out the window when St. Joe's took over North Medical, and then Trinity took over St. Joe's so now it takes 6 weeks to see a doctor for a sinus infection, and WellNow clinics don't actually have doctor doctors as they are just PA's who let me go for two months with multiple spinal fractures on ibuprofen and muscle relaxers without doing an x-ray, so now my hip is out of commission with what is called "referred pain" and yadda yadda yadda...
Long story short-does anyone know of any actual doctors left in the area? Like ones who learned the medicine aspect of their jobs? Not the ones who believe in holistic healing properties of Korean magic stones under the tongue for cancer or other nonsense, but medical professionals? It would be greatly appreciated and favor will be paid back. Or just find Ronald Kameny for me.

I feel like we all benefited from the series and I still go back and read them from time to time. However, the series kind of died off and a lot of specialities went unanswered for. Personally, as someone going into IM, I would love to learn more about Nephrology, Rheum, PCC, etc. Is there anyway we can work with mods on r/medicine, r/residency, and r/physicians to complete the series? I think it would be extremely helpful for a lot of us that didnβt /wonβt get exposure to a certain specialties during med school/training.
Just a thought! If there is enough interest then we should get the ball rolling asap!!

I work in the manufacturing sector so we frequently have these discussions about better machines and AI. Wondering what it would be like in other area.
I know very, very little about the health care and health insurance industries, but I've been reading a little bit about HMOs, PPOs, and POSs and am trying to better understand the difference between the three.
I understand that HMOs only cover in-network care and generally require that patients have a primary care physician. I also understand that PPOs cover out-of-network care and don't require a primary care physician, while POS plans cover-out-of-network care (like PPOs) and require pcps (like HMOs).
According to this article, people covered under HMOs need referrals from their pcps to see specialists. I assume that people covered under POS plans also face this referral requirement to see specialists, but the article doesn't explicitly say whether or not this is the case.
Can someone clarify? I should note that I'm not currently looking a health insurance plan, nor do I know anyone who is looking for one. The only reason that I'm asking this question is out of simple curiosity.
