Male sexual function can be maintained without aromatization: randomized placebo-controlled trial of dihydrotestosterone (DHT) in healthy, older men for 24 months

Introduction: Male sexual function is highly androgen dependent but whether aromatization of testosterone (T) to estradiol is required remains contentious.

Aim: This study aims to investigate the effects of selective estrogen deficiency induced by a nonaromatizable androgen, dihydrotestosterone (DHT), on sexual function of healthy middle-aged and older men.

Methods: Randomized clinical trial of daily transdermal DHT (70 mg) or placebo gel treatment in 114 healthy middle-aged and older (>50 years, mean 60.5 years) men without known prostate disease maintaining selective estrogen deficiency for 24 months.

Outcome measures and analysis: The end points were responses to a psychosexual and mood questionnaire completed before, at 3 months, then at 6 monthly intervals during and 3 months after study. Data were analyzed by mixed model analysis of variance for repeated measures using age and body mass index (BMI) as covariates and including interactions of treatment with age and time-on-study.

Results: DHT treatment increased serum DHT with complete suppression of serum T, luteinizing hormone, follicle stimulating hormone, and estradiol throughout the 24-month study resulting in reduced spinal bone density. There were no spontaneous complaints, or discontinuations for, adverse effects on sexual function during the study. DHT administration had no effects on any of 33 measures of sexual function and mood, apart from a mild, but significant decrease in overall sexual desire, which was reversible after cessation of treatment. Increasing age and less often increasing BMI were associated with significant decreases in most aspects of sexual function.

Conclusions: We conclude that aromatization plays only a minimal role in maintenance of sexual function in healthy eugonadal middle-aged or older men, but age and obesity are significantly associated with decreases in most aspects of self-reported sexual function and satisfaction. The dependence of male sexual function on aromatization may be conditional on age and obesity and can be overcome by a nonaromatizable androgen.

Many people say that this is the only study on the topic, therefore the result is unclear and possibly false. I disagree. There are no flaws in the study design, and a clear relationship between Creatine supplementation and DHT was found.

This is something you should know about if you are looking into supplementing Creatine. Also keep in mind that DHT isn't necessarily bad if you don't mind the side effects. It can enhance sympathetic drive (make you stronger) and do more things that androgenic hormones do. Hairloss and prostate enlargement are some of the big concerns however, and although Creatine hasn't been studied directly for those issues, it would be very prudent to speculate it affects them negatively through the primary mechanistic hormone in both issues, DHT.

Creatine has also been found to have some other benefits like lowering cholesterol a bit, lowering homocysteine and triglycerides and improving muscle strength and brain function in both healthy people and elderly.

Dihydrotestosterone + 3A Androstanediol

I have been transitioning medically for 2.5 years now. I am 28 years old. I take 0.2ml of 40 mg/ml estradiol valerate every 5 days, or 8.5 mg of estradiol every 5 days. My blood tests just got back in and I wanted to ask some questions.

Some hormone unrelated results.
My White blood cell count is 11.8 thousand/uL
My absolute neutrophils is 8118 cells/uL

These seemed a bit high versus the reference range but I am not sure?

Some hormone related results.

My Estrone sulfate is 10673 pg/mL
My Dihyrotestosterone is 21 ng/dL
My 3A Androstanediol is 334 ng/dL
My FSH and LH are under <0.7 mIU/mL and < 0.2 L mIU/mL
My progesterone is 7.7 ng/mL
My sex hormone binding globulin 125 nmol/L
My testosterone, total, ms is 20 ng/dL
My estradiol, free is 7.72 pg/mL
My estradiol is 519 pg/mL

My concerns are DHT and 3A androstanediol. I don't know how to read the former and DHT seems too high. I also don't know if that is a good or bad for free estradiol and my estradiol seems to be quite high versus my last blood test when it was 384 pg/mL

I just discovered that heavy weight exercises, especially legs, increases the production of DHT (dihydrotestosterone). Now I'm too afraid to go to the gym anymore.

I'm 21, pre-everything, and hopefully starting HRT in a month due to super long wait times. Around 5 months ago, I started going to the gym at least twice a week, solely focusing on my lower body. I've been doing squats, hip thrusts, split squats, hamstring curls, leg extensions, and hip abductions, all to look thiccer.

In the past 4 months though, I've noticed the beginning of what I believe is male pattern baldness. I've been taking Finasteride for the past 7 weeks, but in the past month, the hair thinning and receding has ramped up. Coincidentally, so has the frequency of my gym sessions.

Today, I did a lil research and discovered that heavy weight lifting, especially leg exercises, produces a lot more testosterone than other exercises, and as a result produces more DHT (dihydrotestosterone), which causes male pattern baldness.

Now that I've realised this, I'm way too scared to hit the gym again. I clearly haven't been taking Finasteride long enough for it to work yet, and any more hair receding will be irreversible as far as I know. What do I do? Is it worth continuing going to the gym, or should I just scrap weights and just do something like running until I'm a few months on HRT and don't have to worry about DHT anymore?

I'd love to hear anyone's personal experience or expertise with this as it's causing me a lottt of anxiety and dysphoria right now over my hairline. Can my hair grow back? Should I stop gym? Is running better? Thanks in advance for any replies!

Did you have your dihydrotestosterone (DHT) tested?

I'm doing a bunch of blood tests to try to get to the bottom of my heath issues including PCOS. I am doing DHEA-s again (normal when diagnosed), just had my free testosterone done (always comes back normal), and was wondering if any of you got DHT tested? I'm curious if it would show high and explain why my testosterone is low despite acne & hair thinning but wanted to see if anyone here found it informative before I add it.

I had both free T and total T tested a month ago and my results were 25 (24 was the max range) and 724 (920ish was the max), respectively. I did not get dht tested, so I'm curious if anyone can tell me if I have high or low dht based off that?

Many scientific sources give credence to the positive behavioral effects, but if you forgive me for not providing a comprehensive list, I'd like to ask you; have you used one of the following (fairly rare) esters?

- Androstanolone enanthate (Anaboleen Depot)

- Androstanolone propionate (Pesomax)

- Androstanolone valerate (Apeton)

- Dihydrotestosterone acetate

These are specifically pure DHT compounds, attached to an ester for the prolonged and gradual release of it. Synthetic DHT-derivatives are therefore excluded, such as Trenbolone, Masteron and such.

Testosterone v Dihydrotestosterone

I've been on dutasteride 0.5 mg three times per week for a month now due to androgenetic alopecia.I've been dealing with the condition for 6 months which has a slow progression rate but it made me depressed that my hair is just going away.So I decided to take it as I was convinced that after reading a few researches with large control group and placebo group size showing that the incidence of side effects are pretty low and reversible.However I got sides.Prior to dutasteride,I had great libido,penile sensation,orgasm quality and erectile function.Since beginning the treatment all of these have reduced drastically.So I've got a few questions;

1.I've always been told that high-end levels of testosterone with shbg and estrogen levels etc. in optimal range will compensate for the lack of dht in the body and I shouldn't experience any sides.Is that true?

2.If that's true than does it mean I have poor testosterone-estrogen ratio or something like that?Because I read posts of people from r/tressless and other hair loss forums using really harsh treatments without any side effects.I'm going to have a bloodwork done soon to see what's wrong with me.

Low dihydrotestosterone (DHT) but normal testosterone and estradiol. Any advice or theories?

Has anyone here fixed this, or have any insights that could help me? If anything I just wanna figure out why it's so damn low.

I'm a 24yo male, 5'9 140lbs. Symptoms: reduced libido, low energy, significant loss of body hair. I can't figure out what's wrong except for my ridiculously low DHT. Which I don't think is a good thing anyways.

My other blood works aren't great, but nowhere bad enough to explain the low DHT.

How did your Dihydrotestosterone (or any kind of T) levels change during HRT?

So I just called my endo and after three months on 2mg of E and 10mg of Androcur my DHT level went down in a major way!
Before HRT, I was at 626 ng/l, now I'm at 132 ng/l O.O

Did you experience changes this big after such short time?

Forgot to ask about other T levels, but I'll have those in a couple of weeks when I'm seeing my therapist where I will get the detailed test results. :)

My estrogen levels was 'off the charts' as well, but in the sense that it was way too low. That's only because the day before the appointment was exactly the 90th day of HRT and I run out, so I planned to take my daily dose after the appointment. So now I'm curious for my estrogen levels in another three months.

I read about androgens in Wikipedia and was surprised to hear that besides testosterone there's also two more androgens called dihydrotestosterone and androstenedione that are also necessary hormones. Yet I've never heard of those two from my trans dude friends.
Are these two hormones also included in the dosage they take, and if not why not?

SERIOUS: This subreddit needs to understand what a "dad joke" really means.

I don't want to step on anybody's toes here, but the amount of non-dad jokes here in this subreddit really annoys me. First of all, dad jokes CAN be NSFW, it clearly says so in the sub rules. Secondly, it doesn't automatically make it a dad joke if it's from a conversation between you and your child. Most importantly, the jokes that your CHILDREN tell YOU are not dad jokes. The point of a dad joke is that it's so cheesy only a dad who's trying to be funny would make such a joke. That's it. They are stupid plays on words, lame puns and so on. There has to be a clever pun or wordplay for it to be considered a dad joke.

Again, to all the fellow dads, I apologise if I'm sounding too harsh. But I just needed to get it off my chest.
