When I first saw the leaks for the Tactical Philosophies, I agreed with the general view that while Kau'yon's bonuses while active were very good, it would take too long to come online. With the Codex leaks that came out, I believe I've been been able to come up with a list that can truly live up to the ideal of Kau'yon to its fullest (no exact points value because its a pain to do manually)
There are 3 essential components to this list. The first component are 2 Stealth Suit Teams and a Coldstar Commander. The second part consists of 3 Devilfishes each with a squad of Breachers. The third is a squad or 2 of 5 Crisis Battlesuit with a fusion/flamer/third weapon load out.
You can deploy very aggressively with this list, threatening a Mon'tka where your Devilfishes move all the way (and get a 9' pregame move with their Mon'tka ability) and deliver the Breachers up close with the Combat Debarkation stratagem. Likely, your opponent will deploy cautiously against this list.
The fun part comes when you realise that through a combination of abilities that allow units to be put into reserves (Manta Strike, the Devilfish's Kau'yon ability, Exemplar of Kau'yon Warlord Trait), you can in between your deployment and before the first turn, put nearly all your units into reserve by declaring Kau'yon. The ones that cannot can be hidden out of sight and are difficult to root out (Stealth Teams).
Now your opponent is left with a deployment that is less able to control the board due to being deployed conservatively. This gives you more time to wait for your Kau'yon Tactical Philosophy to come into play (as well as keeping ALL your striking power safe).
The Stealthsuits and Coldstar Commander also give you the option to drop an entire Crisis Team even on the first turn through the Homing Beacon upgrade and the Positional Relay respectively. It seems like you could use the Coldstar's once per game reposition to anywhere on the map in conjunction with the Relay to drop your Crisis Team anywhere on the map even on the first turn.
For extra spiciness, I considered bringing 3 Hammerheads if playing Farsight Enclaves, or a single Ion Riptide if playing Bor'kan to really add to the indimidation. You can cow your opponent into hiding with them, then use the Exemplar of Kau'yon Warlord Trait to either redeploy them or put them in reserve. The ability to declare either Mon'tka or Kau'yon after the roll off for first turn will influence your decision (you can deploy aggressively

55 year old male, TKA 5 weeks out. Very litte to no progress 2/2 to pain with a couple trips to the ER for pain the first 3 weeks. Pain medication had apparently not been doing well for the pt but seems to present less irritable for sessions as of late.
Pt has 20 extension and 60 flexion. Patient is very irritable to PROM stretching and I have pushed hard during sessions and have gone soft, but to no avail.
I’m worried the MD is going to reem me a new one since I have only sent progress notes and not made any phone calls.
I start off with AAROM on the stepper for 5-10 minutes, take measurements, then do hands on patellar and scar mobs, also attempt to do PROM flexion and extension as tolerated at this point. Then I do quad sets, seated EOB with foot on slider AAROM knee flexion with belt assist then extension. Have been progressing to more closed chain lunge position to promote ROM and TKE but still not feeling very positive about results.
UPDATE: Was unable to get ahold of RN or Doc via phone today as no one answered so just faxed my daily note. No change in ROM today.
Started with stepper then checked ROM to be consistent. Then I did hot pack around knee while prone over table with leg hanging to get hamstring stretch. Seemed to tolerate it okay. Then I wanted to focus on quad activation so I propped the therapy table into part incline to get his heel sliding with gravity. When I got him to this point his adductors started to guard and cramp up. So put him into SAQ position with NMES onto quads with emphasis on quad activation for hamstring inhibition. Adductors were still cramping and continued to until the end of session.
Patient does have history of low back pain so i palpated L4/5 from supine and pt had referred pain to back of knee, sort of sciatic distribution.
While there are probably multifactorial reasons why I am not making progress, I was a little frustrated after this session because I had all this good advice.

I am hoping to hear some stories about recovery and how that was for you, especially those with leg amputations.
My mother had a through the knee amputation that was unexpected in August of 2021. She has had a few (3?) different prosthetics over the past few months and has been going to physio, but COVID adds some barriers to this (immunocompromised). When she does have a chance to go, she works very hard and the physio has told her she is making great progress. During one of these appointments, she even walked for about 30 minutes between the balance bars!
However, it has been a challenging road, as I have read it has been for many that have experienced amputation. She expressed to me that she does not know what she should be expecting. For example, she says that her prosthetic just feels wrong and uncomfortable. But she doesn't know how much of this is "normal." She rarely wears it and while at home spends her time in the wheelchair.
I think there is a feeling of isolation which makes total sense because family and friends have not experienced this themselves, so there is only so much "understanding" they can give. I am very disappointed that no one has connected her with a support group and that is part of the reason I am reaching out here today. Although it is something I am looking into through our health services (here in Canada), it would mean a lot to me if those reading this could offer some real stories about how this journey has been for them. She is in her 60's which I think offers a different generation perspective on a sort of "just pull up your bootstraps and carry-on" sort of attitude. But this is hard.
Any advice about prosthetics and comfort levels? She does experience phantom pain every now and again, but it seems to be getting better. How about the learning curve of having a "new leg?" If you could tell yourself something when you were going through the beginning months of amputation and prosthetic training, what would of been helpful for you?
I want to do anything I can to help support my mom. I love her so much and this has been so difficult for her. Any stories or kind words that I could read to her and share with her would mean so much. Thank you!! I hope whoever is reading this has had a great 2022 so far. 🌼

It’s been about 3 years since I worked acute ortho and saw total joints immediately after surgery so I’m looking to hear from more experienced clinicians!
My father in law just had a TKA yesterday & came home last night. I’m here for the weekend to help them out. First of all, I’m not used to having TKA patients come home so early, that’s how out of the game I’ve been I guess. I’m used to the typical exercises: heel slides, quad sets, ham sets, short arc quad, SLR immediately post-op. His PT (one session to clear for dc) at the hospital told him he could skip those and go right into long arc quad & assisted flexion in sitting. My in-laws don’t remember specifics of the PT visit of course because it was quite rushed. The PT told him to avoid stairs for a few days.
Is this the new way?! He starts OP PT on Monday but I’m just trying to help them in this short period between surgery & OP. Thanks!
Hello there. Im a entry level PT and I work at an inpatient orthopedic institute. Today I saw a 78 yr old pt in pretty good shape who had his second uni TKA done. He reported he misplaced his FWW from his previous sx and did not want to buy another one in the hospital and was pretty adamant about doing fxnal mobility with SPC which he said he uses all the time. So we practiced gait trng, car, curb, and toilet transfer, and bed mobility with SPC and he was SBA. I also issued a gait belt and DC him home with assist from spouse with HHPT to follow up tomorrow. Was just wondering if anyone would have done anything differently?
New attending here. In my practice, surgeons are doing more same day discharge / surgery center TKA. I am trying to get a sense of how much volume is needed for spinal. In residency, surgeons were very slow and everyone stayed in house so I always dose close to 2ml 0.75% bupivacaine, but I want to cut down on excessive recovery time. Currently doing 1.4-1.6ml (patient dependent) but unsure if this is still more than needed while avoiding GA conversion.
Technique is narcotic free 0.75% bupivacaine plain spinal + propofol MAC, with adductor single shot block after dressings. Typical surgeon skin-to-skin probably around 50 minutes with about 30 minutes of prep/drape time. Appreciate your thoughts.

Hi guys. My boyfriends mom is getting a B TKA right after christmas. I thought for part of her xmas gift this year i could gift her things to help with recovery. Currently my thoughts are getting her a wedge pillow and icing aids. Also want to get her some small stuff like body wipes, dry shampoo, adult coloring books, etc.

Thyssenkrupp (TKA GY) showed strong results for the 4th quarter of fiscal year 2021 and made a statement, if not sensational, then at least noteworthy.
Let's start with the results.
The company's revenue grew by 65% yoy. At the same time, EBITDA went up and amounted to $ 585.7 million, and the net profit was at the level of $ 93.6 million, compared with the loss a year earlier. Excellent numbers driven by the rise in steel prices in the second half of 2021.
Many expected the company to announce special dividends from the money it raised from the sale of the elevator division. However, TKA chose to keep the intrigue, but literally stunned the market with their plans for a spin-off in the steel segment.
This is a significant part of the group's business (about 25% of the revenue). However, for some time, the segment has been generating losses, and only this year, thanks to the strong steel market, it reached positive EBITDA.
We are inclined to agree with the management of TKA: the plans to spin off Thyssenkrupp Steel Europe look logical. True, this is planned to be done only in 2023, and until then, a lot can change on the market. Nevertheless, we believe that strategically this is the right decision.
On the other hand, spin-off can turn into a headache for minority shareholders. This is especially true for Russian investors, who, having received shares in the new company, may be deprived of the opportunity to conduct any transactions with them on the market. Recent examples include the spin-offs of IBM and Technip.
We have included TKA shares in one of our portfolios this week. The stock performed well on the news about the IPO of the hydrogen business and on strong reporting. As a result, the position grew by almost 12% - this is an excellent result in a few days.
So far we have decided to take profit, but we leave TKA shares in sight. An interesting company, traded on EV / EBITDA at 1.5x, and the topic of special dividends remains open.

I would like recommendations from other therapists as to an odd case I have. I am a new-ish therapist in a clinic by myself, so no one to brainstorm with.
I have a patient who is in their 90s. After receiving a TKA, they contracted COVID, and they did not receive any form of rehab during their two week quarantine. The patient is now two months out from this surgery, and after an extended inpatient rehab stay has come to me, a PT at the ALF they reside at. When I evaluated earlier this week, their AROM was -44 from extension prior to any treatment, so this is significant. They are also displaying what I can only describe as a flexor pattern movement, like what I have seen in post-CVA patients, where as I perform manual therapy or they do exercises, the whole leg flexes (not just the knee, hip flexion, knee flexion, ankle dorsiflexion). I've never seen this type of flexor pattern outside of patients with spasticity post-CVA, and they have no history of CVA.
My treatments have consisted of: manual stretching of hamstrings and gastroc, deep tissue massage to hamstrings, gastroc, and quads, PA and AP mobs of tibio-femoral joint and fibular head, knee extension AROM and isometric holds, quad sets with overpressure. They are being seen 4x/week.
My questions are: is there anything else I could be missing from my treatment? Has anyone else observed this flexor pattern in TKA patients, and what is the best way to address it? Lastly, I have never seen someone so far out from a TKA with this little knee extension, and I'm unsure how much I should even expect to gain at this point.
