Has anyone recover from inferior alveolar nerve damage?

It has been 3 weeks since I got all 4 of my wisdom teeth extracted. My chin and bottom lip on the left side still feels numb. It has improved a bit, however still not at 100%. When the nerve was initially damaged after the extraction I could probably only feel 20%, it is possibly at 40% now. I am not sure if any of it has to do with the fact that I am getting use to the feeling of numbness in that area.

I am wondering if anyone here has fully recovered from nerve damage, if so, how long was the recovery? I do not want to lose hope.

15 Years with Inferior Alveolar Nerve Damage

I received permanent inferior alveolar nerve damage approx. 15 years ago during a wisdom tooth extraction. I left medical care about 8 years ago after I was informed the associated orofacial pain had sensitized. I try to keep up with the medical literature for advances, but there don't really appear to have been many. Looks like this is because permanent injury is rare, but maybe also compounded by folks like me getting lost to follow up.

So, in case it might be helpful...anyone have any questions about what it's like to have permanent IAN damage and/or neuropathic orofacial pain?

And, just shouting into the void here, any other permanent IAN damage unicorns out there?

Dentists: Inferior Alveolar Nerve

So two weeks ago I had my wisdom teeth removed. My lower left side had some nerve damage (IAN). I am still recovering from that, and still have a very slight numbness on that lower left side, mainly my front left gum, lip, and part of my chin.

I am set to go to my normal dentist soon to get some cavities filled on that lower left side. Should I wait until my inferior alveolar nerve has completely healed before they numb it again? Or do they even numb that nerve for basic dental procedures? I don’t want to delay the healing of the IAN by any means.

Is this wisdom tooth close to the Inferior alveolar nerve ? Risk of nerve damage?

So, i never discussed with my dentist the possibility to remove this wisdom tooth, but I am wondering is it close to this nerve ? I see a line under it, which seems a bit close, and there is another one which goes straight to the teeth? I'm taking about my third molar on the right side, bottom one. Pulling #31 in a month, odds of hurting alveolar nerve?

I can't find anything on "Dr. Google" about this risk, and my dentist just re-assures 'oh, not very often' etc, so I figured i'd ask you guys. My wisdom teeth were pulled no problem, but I remember there was a 5%-10% risk of nerve damage.

What is the % for complications, in your experience, for lesser molars?

Question on pulmonary embolism, A-a gradient, hypoxemia, and alveolar hyperventilation

https://preview.redd.it/gdurdv3ojz181.png?width=2732&format=png&auto=webp&s=eb1be0da965899e59585d2c3d6ff78d88719c791

Consider this image.

  • A1 and A2 are alveoli
  • a1 and a2 are their respective capillaries.

Let's say a1 is completely blocked by an embolus (yellow drawing).

  • That implies infinite resistance -> blood is redirected towards the other alveoli. So flow through a2 is increased.
  • Now there's a lot more blood that needs gas exchange with A2. Given that blood in a2 doesn't spend a large amount of time with A2, it's safe to assume that gasses won't have a chance to equilibrate between A2 and a2. So, with euventilation, I'd expect hypercapnia and hypoxemia.

Now, let's say the patient is hyperventilating.

  • Why does it seem like CO2 has a much easier time going across the A2-a2 interface than does O2β€”so much so that hyperventilation cause respiratory alkalosis?
  • Why does PaO2 does not equilibrate with hyperventilation? The concept of hemoglobin "capping" doesn't make sense to me here. You have more blood going through a2. More blood = more hemoglobin per unit time. It seems like O2 just has a harder time dealing with diffusing issues. Is that the correct logic here?
  • Lastly, supplemental oxygen does not work with right-left shunting, and I get that. But, I'm trying to see how supplemental oxygenation would work here. A greater O2 gradient -> faster O2 exchange. Is that the idea here? Is that that the "effective" O2 gradient is greater with O2 supplementation than with hyperventilation/

Thank you.
