(I'm alive! Read my first post if you haven't yet, for background/FAQ)
I had non-binary (penile-preserving peritoneal pullthrough) vaginoplasty with MozaicCare a bit ago! This post covers pre-op & the first week, before getting the packing out.
Note that I'm not going to 1) go day-by-day, nor 2) cover stuff that's the same as all the other vaginoplasty surgeries (e.g. what to bring, what packing is like) unless people ask about it in the comments.
I'd like to first clarify a few answers to questions in the first post:
They estimated 8-15cm of depth for me.
About 1-2cm inside the canal.
There was a typo - it's actually the normal dilation schedule (3x daily) not the schedule I said in the first post (1x daily).
She tries to make it just a 1-part surgery, but due to swelling & such you're probably going to want a labiaplasty.
Standard & uneventful pre-op appointment, where I clarified that I wanted her to not discard tissue that could (in the future) be used for the labia.
There are 5 stomach incisions, one in the belly button and two on each side (symmetry, yay!). Then there's the groin incisions, mostly hidden beneath the packing.
Some hopefully useful notes:

This post covers week 3 post-op, where I get out of dangerous-complication territory, get off the rest of my meds, and head back home.
A disclaimer about my experience post-op: in addition to a very high pain tolerance, I've been recovering much faster than expected for my surgery. Don't expect/assume that your recovery would be the same.
Also...there's advice out there saying "don't care what you eat, just eat what's yummy/comfortable, blah blah blah". That's objectively shite advice and I'd strongly advise against that. There's never been a more important time to eat well than when you're recovering from major surgery. Your health & microbiome are completely fucked post-op and need to be cultivated from the ground up. I can't believe people don't make this a top priority.
The first post-op checkup (1 week after surgery), I had the packing taken out. The 2nd checkup (a week after the packing was out) she inspected everything to make sure the graft was secure, bloodflow was returning, no wound separation, and new skin growing. She also pulled some stitches that were poking out, removed bits of dead skin, and snipped some built-up protein gunk.
Not sure what people want to know, but here's some highlights:
My activity level went way up after the 2nd checkup. I had been cooking and such since the day after my packing came out, but I then started cleaning, baking, and moving more freely. Still spent most of the day in bed though. I probably spent ~3-4 hours a day out of bed.
Figured out pads finally! The normal daily pads (size 1 & 2, lightest absorbancy) work fine but are a bit bulky obviously. I'm trying panty liners now and they seem to work but I'll have a concrete verdict next week. They might not absorb enough.
No post-op blues yet. Apparently it sets in at ~2-3months.
There was alot of new, pink skin growth and dead skin/protein shedding. The discharge is gross. Think "nightmare loogies".
Dilation actually got a bit harder before it got easier. I think it was the higher activity level increasing the swelling for the first few days, stopping all medicine, and feeling returning.
By now dilation is pretty painless after it gets in. I'll probably start sizing up to the blue one tomorrow. I can already see how vaginal stimulation will be nice since it goes through the penile nerves/muscles.
I'm
(obviously, throwaway. But I like the username so much I might use it more.)
I'll be having "Penile-Preserving, Laproscopic Peritoneal-Pullthrough GCS" at MozaicCare, with Dr. Heidi Wittenberg, in around a month. Yep folks, the peritoneal hype train just keeps coming!
The peritoneal tissue will be used for the vaginal canal, and the scrotal tissue for the rest. My penis stays as it is. It's 1-stage surgery but a labia revision afterward is on the table.
This post is 0) giving back to yall, because figuring out this stuff all really sucks without this sub & /u/HiddenStill, 1) to encourage me to discuss post-op stuff afterwards, and 2) to answer questions and address alot of nonsense to be found online about PPT. Because, goddamn, the amount of uninformed "information" flying around about PPT is pretty bad.
Before I heard that non-binary transfemme GCS was possible, I was pretty ambivalent about it. Of course, I'd much rather have been born with the traditional female parts, but after my orchiectomy last year, I didn't feel like it was worth going further with current GCS tech. I did/do have bottom dysphoria, but it was about not having a vagina, not about having a penis.
Hearing about non-binary transfemme GCS was mind-blowing, frankly. It was all of what I wanted. I am transitioning to binary female (albeit gender nonconforming), but I have my reasons:
This is the part that gets everyone's panties in a bunch.
There's been alot of noise about Dr. Ting & PPT, as well as some other people. It seems like, with most surgeons, the problem they have with PPT is the laproscopic part. A surgeon needs the training & equipment to use a special robot to go through your belly-button and do crazy stuff in there. Since GCS traditionally isn't done with robots, lots of surgeons don't have that experience/training. Also, there are different types/extents of PPT - as the surgeon for specifics.
As to the "surgeons don't recommend it because there's problems with PPT": hopefully you've read all about how PPT has been used successfully throughout the last few decades for cis wome

This (belated) post covers week 6 post-op. It's about nerves, dilation, stitches, and moving past the "surgery" mindset.
I can sit down without feeling pressure, unless I lean forward. I think part of this is just me regaining my (substantial) leg/butt muscle but also less swelling.
Also the lopsided swelling/healing has started to fix itself, phew. Not completely but hey it's progress.
It's all slooooowly stretching out again after the retraction phase. My insides are starting to re-accommodate it all. I can also dilate right after extending walking sessions without much additional trouble. Still on purple, though.
Oh boy - I think I broke a stitch or two in the last couple weeks. I didn't notice it because I assumed broken stitches == separation. But yeah, I'm pretty sure now that the stitches are starting to come apart/loose/break/whatever. But because it's all healed pretty well, they're stuck inside instead of popping out or whatever.
Regardless, they're starting to annoy me. I want them out. Now. They restrict my range-of-movement, which is kind of a Big Deal with my ASD. The pain of them poking outside/inside me doesn't cause near the suffering as being hindered. And I feel like my healing has got to the point where I don't need them anymore. So, next week I'll check in and see if I can get permission to do something about it.
Feeling is officially come back! Most of this involves awkward pain and itchiness. Like, I can feel the ends of the broken stitches under my skin poking inside if I mess with stuff in certain ways. It's pretty bad pain I guess? I'd put a second-degree burn on your palm at a 5, and this is at the same level. Like, bad enough you have to address it.
Also, I've really been noticing how I feel "fuller" down there. The feeling inside & outside is returning and I guess my nerves are getting used to all the new stuff. It's pretty cool & gender-euphoric.
Same as before. No update unfortunately. There's a last spot of dead skin from the graft that's going away but it's basically just a tinted clear stickiness. There's alot of it. Pretty nice.
This last week I've been getting past the "surgery" mindset (as shown by my late post!). I think it's healthy mentally, to be switching back to "normal life" mode. It helps that I'm not working yet ofc. I also am 99% sure I'll dodge the "post-op dep

This post covers week 2 post-op - basically just dilation, douching, healing, and avoiding complications. An important note - it's the same post-op schedule as normal vaginoplasty. The surgeon thinks that the peritoneal tissue should mean less dilating & no douching, but to be safe she's putting everyone on the same post-op treatment until she has more data.
Pretty standard - 3x daily, 10-30 minutes. I use the smallest one (purple) and my depth is at the second dot from the handle. I haven't measured it but from eyeballing it I'd say 5-6inches. I'm not really interested in working up to the super wide dilators (seriously, the orange one is ridiculous) but I suppose I should at least go up to the blue one.
It's a little annoying having my penis & scrotum in the way of dilating. Thank god I have a hand mirror.
It takes ~2-3 minutes (maybe less sometimes) to get the dilator all the way inside. Every time it's a little different. The first several times were scary, as you didn't really know if you were breaking something or not. By now I'm not anxious but it's still uncomfortable.
It's surprising how much I can feel inside, even now. The peritoneal tissue doesn't have tactile sensation like your outer skin does, but you still feel the dilator the whole way in. It's much more sensate than your butt. It reminds me a bit of putting things in your mouth actually haha. I'm excited to learn fine muscle control in the future once things heal more.
1x daily for week 2, every other day for week 3, and then once a week after that. I think at 3 months I'll try stopping douching if the secretions are acidic enough.
Douching is easy and painless and actually feels a bit tickly.
Week 2 was dubbed the "swelling week" by Dr. Wittenberg. Apparently this week is the worst for swelling & has the highest risk of wound separation (eek!). Luckily nothing happened to me.
Besides wound separation, the sight of my vagina was probably the most frightening part of it all LOL. You see all the post-op pictures and are prepared for it, but you still can't believe "that" will heal into something that will look nice.
Funnily enough though I'm already happy about the surgery. It's very annoying having to keep the scrotum around, but my genital dysphoria is already lessening. I can tell this was the right thing to do. I just can't wait for the

This (belated) post covers month 2/week 8 post-op. There's big news about swelling & stitches!
I don't know exactly what happened but outer swelling went way down in the course of two-ish days. The entrance also contracted (completing the scar contraction phase) but has been getting better.
It's still lopsided though! I think it's actually granulation tissue all on one side of the canal though, after looking/feeling at it a bit.
I'm happier about the aesthetics now too. I didn't realize how swollen the area had been until now.
The outer contraction peaked maybe a day or two after my week 7 post. A couple mornings it took probably 5 minutes to get the full girth of the dilator past the entrance, but after that it was easy since luckily the canal had already went through the contraction phase in earlier weeks.
I might have "lost" a bit of length due to all the swelling going away too. By a "bit" I mean like 1/3 a dot on the dilator.
The swelling going down meant that there was even more suture unravelling/hanging out, so I finally decided to get some of them out. I spent 2 hours with a hand mirror, tweezers, and clippers, trying to figure out where/how to snip them. They're basically invisible, with teeny little knots/loops, so I ended up having to feel my way...with pokey metal things. Not fun.
I still have some left in, in the inner labia, but those don't bother me too much for now (and I can't figure out where the knots/loops are that are keeping them in).
Feeling has come back to the bottom rim of the entrance completely but the rest of the inner labial-things are still numb to touch. I can feel the nerves healing though.
Granulation tissue seems to be slowly going away! And the last of the protein stuff has been gone for awhile now.
I think I can start adding light exercise - once I get over this cold I'll start doing a relaxed yoga session daily.
Mentally: everything's fine like it has been. No post-op blues. I have my mental energy back for dealing with ASD-related stuff, I guess.
The last month has been pretty good. Dilation has turned out to be a nice routine and helps structure my life right now. I've scheduled my 3-month check-up a bit early so that'll happen in a few weeks.
As always, feel free to ask questions via comments or DMs.
(see post history for previous entries about this surgery)
This post covers week 5 post-op. It's about healing, exercising, and dilation.
Also, something that's not mentioned really at all - there's no word on what GCS does to the musculature down there. I doubt we have the same musculature as cis women, but I guess we assume GCS fixes it? I don't think I've read a single thing about this, and I hope everyone starts asking this to their surgeons. It's too late for me (I'll ask at my 3-month checkup anyways) but hopefully it can be a consideration in the future.
Continues to go down! It's also starting to stay down even when I've been up & active for hours. I'm basically on my feet as much as I was before surgery, but I recline on couches instead of sit in chairs (most of the time).
The stiches are coming out, too >.< . I'm not interested in trying to snip it because I don't have feeling back on the inner labia yet (where the stiches are). So I just have to live with them poking me. Oh well.
I was doing some blue dilator, but then the retraction phase hit! It's no lie - in the 2nd/3rd month everything gets tight as your insides heal. In the space of a couple days it went from nice and easy to be pretty slow (e.g. 1min to get in, to 3-5min). It's not actually painful, just really tight. Feels like sore muscles. So, I'm stuck at purple for now.
Also, fun fact - I regained the length I thought I had lost. I mentioned thinking I might have lost some awhile ago, and just decided to try dilating a bit longer (e.g. ~20min instead of ~10) and voila, I'm back! Possibly even a little deeper.
Finally, I can get those endorphins! Right now I'm at a daily ~90min hike in the local trails. I'm hoping to start doing more in a week or two.
By now I'm confident that higher activity levels is really helping the healing. The swelling doesn't even get bad after all this walking.
Healthy pink flesh is conquering the granulation tissue! Slowly but surely. I still have some protein coating but it's been disappearing at a healthy rate. It's still lopsided because one side had a tonne of granulation tissue, but it's been getting better.
Feeling hasn't changed much, just more nerves zapping. I have no feeling around the canal's entrance yet.
I'm not having pee/bladder issues anymore! Probably was resolved with all the muscles healing.
Another thing people don't mention - post-op sleep issues! Gah! This is gett

This post covers week 7 post-op. It's about...healing & dilation I guess.
Hasn't changed noticeably I think. I keep dilating right after being on my feet for a couple hours which is never a great idea. I don't recline/lie down any more than I would normally now.
I've found that I need less lube than I think I do. I noticed alot of lube gets pushed out and drips down before I even get it in all the way. From what I can figure out, I actually use the natural secretions most of the way because the lube gets stripped off pretty early. I've been trying significantly less lube than I was instructed to use and don't think I'm hurting anything, and I'm wasting less. We'll see!
Totally forgot about these for some reason. Some of the stitches have came out a bit more and they bother me much less than last week. I think I might try to pluck some out completely because they're really starting to unravel.
The good healthy pink stuff continues to spread! There's also some new granulation where protein had been but hopefully it'll start going away.
Also, wow my muscles haven't recovered as much as I thought they had. I can walk, sure, but I'm still significantly weaker. I think I'll take a stab at yoga later if hikes this weekend go well.
There's still a bit of protein coating (maybe sloughing tissue still?) inside that I saw last time I used the mirror (a couple days ago).
No sign of the post-op blues. I've been really "busy" (relatively) getting back into some higher-energy activities and finishing catching up on the bureaucracy of life. I can't believe it hasn't even been 2 months! I feel guilty not working now, but I planned for 3 months recovery, and I don't know how I'd swing the dilation thing if I had to go to a workplace.
(see post history for previous entries about this surgery)
Woops, skipped a couple weeks of updates...this post covers weeks 9, 10, and 11. There's no special stuff, just the same topics of swelling, dilation, healing.
I don't think this has changed much, which is fine, because I don't notice in in everyday life. I guess from here on it's just the last bits of swelling that are slowly going away.
I've "re-stretched" things by now which is nice. I was only doing twice/daily for a week due to business and I could feel it get a bit harder to dilate.
Still on purple though! I figure I'll size up when I get back home in a couple weeks.
There's lots of nice pink peritoneal tissue in the mirror! Thankfully the granulation tissue never got too bad.
I actually haven't tried more intense exercise than hiking yet (cuz I'm busy nowadays) so that'll have to wait till December as well.
Nerves are going a bit crazy sometimes, some feeling has came back in weird ways. There's places I can touch and random-ish nerves will just light up, like this spot at the top I can press and feel it all the way to the tip of my penis. I'm assuming things will normalize but it's pretty fun(ny) for now.
The discharge/lubrication/whatever is real, folks. 24/7. It's definitely influenced by "things" but I'm not sure what the factors are yet. I think being more active (e.g. when I hike) means more damp~wetness. Maybe also how hydrated I am. It'll be interesting to see the change when I go from dilating 3x/day to 1x/day...I had previously assumed that a significant portion of the discharge was leftover lube from dilating but TBH now I don't think so.
100% fine. Don't really think about it too much TBH, it's just a part of life now. And I'm glad for it, especially since obviously 1) it hasn't healed all the way 2) I haven't had the part 2 (labiaplasty) yet. But even if nothing else happened I wouldn't regret it.
I'm having my 3-month post-op checkup soon, where I'll schedule my labiaplasty!! Excited.
Also I've tentatively decided that I will post pics after the labiaplasty, so y'all have that to look forward to.
I'm a relatively new player (2 years) but I'm struggling with getting a consistent bent elbow on my drives on the pullthrough.
When I have the disc at max reachback (with back at target and arm 90 degrees to shoulders do I 1) pull the disc on a straight line across (and close) to the chest while turning the hips/shoulders or 2) actively bend the elbow until the disc reaches the chest while turning the hips or shoulders?
Are there specific things you all think about when bringing the disc to the chest with a bent elbow? It seems like when I purposely try to bend the elbow and bring the disc to the chest I lose a lot of power (when I know I should be getting more power when doing this).
I'm thinking that this could be a breakthrough moment in my swing but am unsure what the best way is to apply the bent elbow. Anyone have any advice or things you think about on the pullthrough?

This post covers post-op week 4/month 1. This has been the first week home, and another week of relatively rapid healing/etc. I'll also cover some "month 1" stuff, including things I might have forgot to mention before.
Swelling seems to be the biggest source of suffering post-GCS, and I think I've addressed it properly so far. This week, the swelling has went way down! I'm not sure why but I'm inclined to think it's related to time and my increased activity level - mostly alot of walking & stairs.
I haven't needed the donut pillow at all the last few days...sitting on hard surfaces & such aren't uncomfortable, but I don't sit too long (<1 hour at a time) and I can still feel stuff being pushed around.
Probably relates to the decreased swelling, but this is also alot easier too! 99% sure there's granulation but 1) it's not getting in the way, and 2) I think some of it is starting to go away. It's basically got to the point where after some initial discomfort I just slide it all the way in. Sometimes it goes in without any trouble at all (when I haven't been active).
I've tried the next size up (blue) once a day these last few days. I can get it in most of the way, but it's uncomfortable & takes long and TBH it's not worth it for me right now. Still having a penis means that there's alot more inside which makes stuff alot tighter. And I worry about the peritoneal tissue being fragile. I'll try moving to it again next week.
One thing that I might not have mentioned - dilation messes with my bladder and the associated muscles. I've always had bladder control issues but ugh. I can almost guarantee that the next time I sit/lay down after dilating, a little pee will squeeze out. It's getting better as I heal & swelling goes down, but it's still annoying.
Like I said above, there's some granulation tissue. Today seems less than the last couple days, but I'll need a few days to make sure it's actually going down.
Discharge is changing from thick opaque white/yellow/green to sticky clear-ish yellow/green. As the protein stuff lessens, the discharge is clearing up. It's not very water-y, but slimy.
I officially just use pantyliners now because they're so much more comfortable than anything else, but I don't mind the discharge that gets on the underwear.
I make sure to do sniff tests daily to make sure there's nothing iffy since I'
That title is a mouthful.
Often, particularly after my missile infantry have been charged by cavalry and are disengaging because of the threat of nearby allies coming to prevent further damage, the enemy cavalry will have models within my unit or on the opposite side of my unit from the direction they are traveling.
I want to walk through the few scenarios so that we are all on the same page and then discuss what problems I perceive, what downsides may exist for my suggested change to the current system, and potentially other solutions if I think of one.
Opening - When the cavalry charge lands
This part will be the same for both scenarios want to talk about.
Normally I will see incoming cavalry, target it with range, and attempt to deal damage before they can impact me. This can include attempting to dodge the charge via dispersing my units, docking on a cliff or using 'Crippling Shot' since the introduction of Ambiorix (I primarily play barbarian ranged).
When a cavalry charge is successful and lands on one or more of my units, my ranged attack order will be terminated. Move orders arent, but because the models are knocked down for a short time that gives an opportunity to issue an attack order on the enemy cavalry for when the missile infantry stand up(I have auto-attack disabled, which is a must for a missile infantry player). At this point the cavalry likely have an attack order on me and I have one on them, and our units are intermixed or some cavalry are on multiple sides of my infantry.
All of that is well and good, and will be the premise for the next two scenarios.
##Scenario 1: Cavalry & Missile Infantry attempting to mutually disengage
So in this scenario, friendly models are approach and the cavalry wants to disengage. They issue a move order, and start receiving pullthrough damage from my infantry, which usually amounts to about 10 damage(Hidden Melee AP Damage stat of missile infantry). The cavalry models can get stuck for quite a while.
If I attempt to issue a move order while the enemy cavalry is making its move order, I will being receiving pullthrough damage from the cavalry, even though both units are disengaging. Because missile infantry are not meant for melee, and many cavalry units will have significantly better stats, I will generally lose a number of models during this period of mutual disengagement, while the cavalry tends to lose few or none.
Now, my goal is not to have the cavalry lose any models. My goal i

I don't want to step on anybody's toes here, but the amount of non-dad jokes here in this subreddit really annoys me. First of all, dad jokes CAN be NSFW, it clearly says so in the sub rules. Secondly, it doesn't automatically make it a dad joke if it's from a conversation between you and your child. Most importantly, the jokes that your CHILDREN tell YOU are not dad jokes. The point of a dad joke is that it's so cheesy only a dad who's trying to be funny would make such a joke. That's it. They are stupid plays on words, lame puns and so on. There has to be a clever pun or wordplay for it to be considered a dad joke.
