Conclusions
There are distinct differences in clinical characteristics and prognostic factors between the early and late death in patients with ARDS due to pneumonia. Early deaths are largely caused by high severity of general condition characterized by cytokine storm and late deaths are associated with early fibroproliferation with coagulation abnormalities. Systemic severity, the extent of early fibroproliferation on HRCT scans, and coagulation abnormalities should be taken into consideration in personalized medicine towards ARDS caused by pneumonia.

My mother was 63, 5โ7โ 160 lbs recently died of her heart stopping days after fracturing 20 ribs in a severe fall, getting pneumonia, and contracting ARDS. She was a nurse and was alert and knew much more about medical things than I so i trusted her own judgement over mine.
However, she died after 12 days in the hospital. Initially we were told she was doing very well after the fall, she wasnt even on oxygen. After just a few days in icu they moved her from icu to the normal hospital and were talking of discharging her to a rehab facility in 4-5 days.
Then a few days later she told me shed gotten pneumonia. A day after she was moved back to the icu and put on oxygen. She was recommended to go on a ventilator and i witnessed once where she declined it - her O2 stats were in the 90s but only when she was on the max oxygen they could give her through a mask. She agreed shed go on one if she absolutely had to but she felt she didnt need it yet and the dr told her she needed to tell them when her stats were going down so they could put her on one.
I wasnt there to witness her death, but i was told she had an emergency early in the morning where her oxygen stats were too low and her heart stopped before they could get her on a ventilator. The doctor said they attempted cpr (despite her many fractured ribs) but couldnt revive her, and she didnt have enough oxygen โin her reservesโ by the time they got the tube in her. I have been so miserable since her death thinking maybe i couldve done something to convince her to go on it earlier and shed be alive today. I just assumed she was in icu and theyd get her on one when the time came.
I have so many questions because it seemed like shed survive this and she didnt and my life is miserable now with regret and guilt. Does anyone know if the doctors explanation adds up? Should my mom have made it on a ventilator in time if she was in icu? How long does it take to get a patient on a ventilator? Should she have been discharged from icu so quickly when she first got there if she was at high risk for pneumonia? Any insight is much appreciated
