Some telemedicine companies not offering tail coverage?

Applying to several telemedicine companies for supplemental income. A couple now are telling me that they don't cover tail coverage? (companies are - "the largest one, starts the 20th letter of alphabet, Doc" and also "'11th letter of the alphabet' health."

Was told that "as long as the company is still around, it shouldn't be an issue."

I want to make sure my chain isn't being yanked, nor shoot myself in the foot by playing with fire (3 metaphors in one sentence, noice).

Wut do? Need money.

Edit: Info from one company-

"While xxx's malpractice policy is a claims-made policy, there is no need for tail coverage because the policy is maintained (renewed) indefinitely. Claims-made policies cover any claims made anytime during the policy period. Therefore, the only time tail coverage would be required is if xxx terminates its malpractice policy, in which case xxx would purchase a tail policy that would cover all claims made during the tail period (usually 3 years).

It is important to note that it doesn’t matter if the physician is no longer working for xxx; claims against that doctor are still covered, regardless, for as long as xxx maintains the policy (and during any tail coverage after that). This is different than many policies because xxx's policy does not cover individual, β€œscheduled” physicians. Rather, the telemedicine policy covers all visits performed on the xxx platform by any physician anytime he or she was working on the xxx Platform – it does not depend at all on whether the doctor is currently seeing xxx patients. And, again, since xxx maintains the policy continuously, covering all visits by all physicians, there is no need for tail coverage for individual physicians. "

If anyone is still iffy about using one of the telemedicine companies to get suboxone, this is your sign to do it!!

I was unsure for months about getting on suboxone to kick tia. I thought it would be too hard to get the prescription, too expensive, too many hassles, etc. I finally decided to just say fuck it and do it and im so glad I did. It was almost too easy to get them.

I made an account on bupe.me, was called back half an hour later, made my doctors appointment later that day, the next day I had my doctors consultation which was less than 5 minutes, and got my prescription from Walgreens the next day. It was so easy and they help you out every step of the way. No drug tests, no waiting in a doctors office, no hassles.

It was a lot cheaper than I expected as well. Especially compared to using over $300 worth of bulk tia a week, it was nothing. It cost me $50 for the doctor consultation and another $10 for my copay from Walgreens for 14 8mg suboxone strips. I spend more on shipping costs for my online tia purchases every 2 weeks than I spent on the entire process to get the subs.

I wish I did this months ago. I have been tapering tia the past week and I am taking my last dose tomorrow morning. I am going to wait until I am in full blown withdrawals, which should be around 8-12 hours later, and then starting to induce subs. My plan is to take 1-2mg every 2 hours or so until I am out of withdrawals. Im not looking to feel great, just to feel decent. Please wish me luck and if anyone has any advice on using the subs or anything related to this process, please feel free to leave a comment or shoot me a pm.

Anyone in a similar situation, I highly recommend to use a telemedicine service and get on suboxone. I promise it is a lot easier to do than you think and you will be happy you did it. Thank you to everyone on this sub for all the support and advice and I hope we can all kick this horrible addiction.

external moonlighting that's telemedicine-based during residency

During FM residency, are there ways to set up external moonlighting that are solely telemedicine-based? I know that moonlighting itself is program/institution dependent but I'm wondering if there are entities that will allow residents to moonlight for them in evening hours or on the weekends via telemedicine? Similarly, are there ways to work with telemedicine services that are app-based? If someone can explain the range of moonlighting opportunities (both internal and external), that would also be helpful. My partner is going into rads and most of the programs he's applied to offer in-house moonlighting where the residents get to chill and they're only responsible for watching out for contrast-induced reactions....that's what gave me hope that there might be some lower-effort opportunities available for FM residents. Would love to make some extra cash during residency from the comfort of my home (if that's even possible). I'm also really interested in abortion, repro care, & addiction medicine and I'm wondering if there are organizations I can work with via telemedicine and be compensated for the work (I would absolutely volunteer my time but being broke makes that hard).

Telemedicine and controlled substances in Pennsylvania?

Hello, I live in the country side and have a hard time finding a psychiatrist without traveling far. I would like the convenience of telemedicine like Done or Ahead or some of the others but I’m worried that controlled substances might need an in person appointment. Is there any place where I can find this info or does anyone have experience with this in Pennsylvania?

I was on adderall from 2007-2014 but my symptoms have gotten much worse and would like to continue treatment but I don’t have a car. So I’m kind of stuck.

I don’t want to pay for Telehealth appointments (I have no insurance) only to find out they cannot prescribe controlled substances.

No idea who is selling here. We are closing down 7.5% from an already incredibly cheap valuation with the overall market getting back strongly from early heavy losses. Major update on Telemedicine

This is a quick and simple post. I just got word from PFM and have updated the telemedicine page on the subreddit's wiki. It appears that PFM has gained interstate licensing for many states, and the wiki page has been updated to reflect this.

If you are a remote patient, or are considering signing up on the waitlist, I would highly recommend that you review your state's status. I've provided a link to the page below.

Why is Adderall available for Telemedicine prescription but not modafinil?

For scale (lower the number the less dangerous):

  • Schedule II: Adderall
  • Schedule IV: Modafinil, Xanax

However, it seems Adderall is currently available via telemedicine. I did a quick search on reddit and there are stories like this that shows telemedicine does prescribe Adderall. On the other hand, I don't see any stories about people being able to get modafinil prescriptions from telemedicine.

Two questions:

  • Why the difference?
  • If you've taken modafinil and adderall before, which one is better for mental clarity and motivation?

I'm so over telemedicine - and why I finally got out (just a rant)

I graduated August 2019 and it took a couple months to land a job. Signed my contract November 2019 for an outpatient psych gig. Credentialing and everything took awhile (as it usually does) and then the pandemic hit us. So when I finally started in April 2020 it was 100% telemedicine. I have been doing telemedicine now for 1.5 years and I am so completely over it. Currently I do a mixture of telemedicine and in person visits, but I would say its 95% telemedicine and 5% in office. Here's what I've noticed over the months of doing this.

Patients do not treat telemedicine visits the same as in office visits, and I certainly do not receive the same level of respect. Yes they are more convenient because you don't have the drive all the way into the office, but the majority of my calls patients are distracted, there are dogs barking in the background, there are kids screaming, people are at their jobs. My personal favorite (sarcasm) is the calls I get from people in their car driving. I make them pull over or reschedule, which has lead to many unhappy patients. The system we use for telemedicine sucks as it is - and office staff is required to call the patient on the phone and check them in prior to being seen on video by me. Half the time patients miss their check in calls and just show up in the virtual waiting room and when I tell them to call the office back to check in they get huffy and mad. I've received multiple nasty messages from patients who were not able to be seen because they were late. But when I have a schedule of 22 patients a day and you show up 10 minute late to your scheduled 15 minutes, it is not enough time for me to do my job and not fair to my next patient if we run over so I make people reschedule. People smoke cigarettes while they talk to me. People are in other doctors offices while they talk to me. People are at the grocery store. I've complained before about this but my office staff doesn't confirm whether patients are located within our state prior to visit, and at this point I've just given up policing this issue even though its the rules of telemedicine.

The bottom line is this - patients are rude, nasty, and noncompliant as it is. I deserve more respect. Respect my time. Respect my super busy schedule. Respect my rules (that have not changed in the entire 1.5 years I've been at this office). And on top of that psych patients are difficult at baseline as well. So I finally landed myself a new job doing all in person visits sp

Fair reimbursement rates as a contractor for a telemedicine company?

Let's say you're considering signing a contract as an independent contract to see patients with a telemedicine company (they take care of everything from insurance, to providing emr, to billing, to marketing, etc etc).

Let's say reimbursement is based on E&M codes submitted based on medicare rates at the 54 locality.

  1. What does reddit feel is a fair percentage going to provider vs the company?

  2. Would you feel the answer to % reimbursement to provider should change based on being a specialist that is known to be in higher demand for more rural populations?

  3. Is this 54 locality a way to reimburse me less? Google search showed that this may be based on 70% reimbursement of typical medicare rates for a specific E&M code

Any answers/guidance definitely appreciated

Medication interactions and telemedicine

Hi all,

I have been receiving ketamine infusions since April. They are often unsuccessful and I really do not have the money. The clinic I go to also provides my psych meds, and I know everyone there, they know me, I know people firsthand who started the therapy before I did. So that is why I stuck with them for so long. But I am losing my confidence in them and it seems there are other routes that people have had more success with.

I haven't considered telemedicine for ketamine therapy until really diving into this sub and realizing that it is legit and safe. I am on a moderate-high dose of lamictal, klonopin, and Lunesta, which I know all work on GABA and inhibit the ketamine from being optimally effective in the brain. My doctors told me not to change anything about my meds during/before/after ketamine treatment, but again, reading through I am coming to the conclusion that those are the main culprits when it comes to some infusions being unsuccessful.

I have reached out to reputable telemedicine ketamine therapy providers. Will they still consider treating me while I am taking those meds? Or would I have to get off of them first before starting? (Which might take forever...) What exactly should I disclose and what is acceptable to omit?

Thanks SO much. This sub has been a life saver and if I could hug every one of you right now I would.

In the spirit of receiving and giving support I just want to add that if anyone needs support I will always try to help if I can. My inbox is always open.

Thanks guys. Hope everyone is having a great weekend.

Here's a two-per of unrelated dumb questions: Specialty medications for cash patients? Buy and bill flexibility w telemedicine?

Question the first: How do cash paying patients go about obtaining specialty meds that are usually on the orders of tens of thousands? I mean, I know there are a bunch of direct primary care practices and patients out there. Do they just, never take these kind of meds? Or is there a necessity to have some insurance plan to cover these? Some kind of work-around if a provider does not want to contract with insurance companies?

Question the second: Buy and bill medications are typically bought up by physical practices and administered there. Is there any feasibility/legality/meeting requirements for reimbursement if this is be done on a telemedicine basis? Could you buy, ship to pt's house, and contract w a nurse to administer in their house, and still buy and bill? Or perhaps contract w their third party location to be administered there??

Question the third (okay I lied about the two-per): Let's say I'm too lazy to start my own private telemed practice. Any telemed companies out there that you know of not forcing minimum work hour requirements, ya know, for the discerning physician that just wants to dabble their twinkle toes in to feel the waters?

The SPACE / De-SPACE

Motion Acquisition Corp merged with DocGo (Ambulnz) in Nov-21 under the new ticker DCGO

They make concierge medicine more widely available i.e. Royal Pains for all

(IMDB, 2016)

Most SPACs are pre-revenue and have pipe dream projections based on profitability many years out but not DocGo. When Motion Acquisition announced the merger back on Mar-21 at $10 they were valued at ~$1B market cap with a 2021 revenue projection of $155M and a positive EBITDA. (Well that was then - read along for how they just TROUNCED those projections since then…)

The Business

DCGO provides last mile health care service which includes -

  1. Medical Mobility services transporting patients between clinical settings e.g. from home to hospital or dialysis center and back (boring but steady business)
  2. Mobile Health services such as vaccinations, testing, would care, imaging performed in the comfort of the patient's home, workplace or other setting such as on a cruise ship i.e. like Teladoc but they do the extra step by providing actual in person services at your home / workplace.
