I have my own theories but I think incest as culturally practised doesn’t provide an easy answer for this so it must be something else. None of these have any basis in real world science and have everything to do with fantasy fiction.
There are strong evolutionary checks on risks of breeding between such close relatives which greatly diminish the possibility of sexual activity. Now, in very specific cultural environments, there can be impetus to override these but in almost all situations, incestuous marriage between siblings was promoted as a duty and followed as such. We never get the impression that the romances among the Targaryens were based on just a strong dedication to duty and tradition, but rather also, a genuine and strong sexual attraction.
Alrightalrightalrightalright!!! After what I thought was a lost cause, I just bumped myself from 108% completion to 109% :D and I did so using the charms: unbreakable strength, sharp shadow, stalwart shell, Grubsong, and thorns of agony.

So I'm raiding the Nordic coast for my homeboy king harlaus. Been doing good with my band of merry men, even won a battle against some nord lord. I'm fucking invincible at this point ain't nobody messing with me. Only one village left that I havnt raided but it's all the way down the coast. I decide to fast travel because I doubt anybody is gonna fuck with me. Make it halfway before I'm stopped... sea raiders.. He's apparently going ti "DRINK FROM MY SKULL!!!" I ain't having any of that shit. I'm outnumbered but I spawn in anyway and set my regular line of horse in charging formation with my infantry and archer holding back. I see those fucks charging towards me so I send my cav in for an early charge.... they got slaughtered. I was onky able to save 3.... 3 of my god damn swadian knights... then they came for my infantry.... they were slaughtered.... I wanted to leave but I couldn't.... I'm getting pelted by throwing weapons and all I can hear is the sounds of my dying comrades and thise damn nords yelling. I get killed and heald captive..... fucking sea raiders.
