My uncle died last year of stealthy Myelofibrosis that was only diagnosed in his autopsy. My elderly Dad has unexplained fatigue, that hits him “like letting the air out of a balloon” most (not all) afternoons. He is working with his doctor to diagnose that, with an eye on MPN.
In the mean time, I got a home genetic test that clearly shows that I have the JAK2-V617F mutation. (It looks homozygous in the 30x reads, but I read there is a weird mieosis-like thing between the short arms of chromosome 9 that might falsify that).
I’m 57, with no obvious MPN symptoms and normal blood tests. (My Dad and I have mild eczema that the JAK2 explains).
I do have unexplained arthritis in my right knee and ankle. The Rheumatologist has ruled out Rheumatoid arthritis and Psoriatic Arthritis.
My ankle has been swollen for ~a year, and is often hot (like gout). I read that PV sometimes causes “gouty arthritis”. But my serum Uric Acid tests are consistently good (5.6-5.8) — seemingly ruling out gout.
Now, I read that is not always the case — you can have localized gout with low Uric acid overall. I asked my Rheumatologist about this. She admits that she has no experience with PV “gouty arthritis”. She does not think I have gout because it is relatively constant and “not episodic”. Tests continue.
My question then — any of you have PV “gouty arthritis” ? How does it act ? Is your overall Uric acid normal ?
Hey all, I (23M) just weaned off of prednisone that I was prescribed for 6ish months, taking 8-16mg a day for arthritis. Unfortunately, my doctors haven't been able to identify which type of arthritis I have yet, but it was triggered by Lyme disease which I have relatively successfully treated over the past year and a half. Symptoms point to RA, but all blood tests including CRP, sed rate, rheumatoid factor, and others have come back negative.
Since I finished the taper, my fingers have been in constant pain and the joints are very swollen. The weakness and fatigue are so bad especially in the morning, I struggle to even open an unsealed water bottle. I wanted to see which products people have the most success with.
I have tried meriva curcumin in the past with no effect. I'd be willing to give one of the ND extracts (probably curowhite) a shot, but I'm also taking Lexapro so I will avoid curcumin until I wean off that next month.
PEA seems really promising, but it's rather expensive and seems to take a month or so to kick in, which isn't ideal in my current situation.
Rephyll is another option, but I haven't seen too much about its effects on arthritis. Would love to hear some experiences with this (bonus points if anyone knows how it interacts with THC, if at all).
Tart cherry might be helpful if it's gouty arthritis (going to request blood work for uric acid when I see my doc next month), but otherwise probably wouldn't do much for pain? I have tried it in the past while on prednisone, so I don't know if it did anything besides slightly lower my HR.
Injuv, black seed extract, sesame extract, 7,8-dhf, and hesperidin seem interesting to me, although I've done less research on these.
So, what does everyone think? What has helped you the most, particularly with finger (PIP/MCP joints) inflammation? Thanks in advance!

Kind of at a loss and don’t know what to do . Started my journey of dieting and working out about 5 months ago . My weight was 315 and as of today 260 So I have been on the keto diet for 67 days and about thirty days in I started experiencing gouty arthritis (have had many previous gout flares) and also had heartburn which had gone away before keto but has came back during. Originally I was doing a low fat and really prioritizing a calorie deficit and it began to get tough to maintain. So starting this diet my general routine was intermittent fast about 4 days out of the week. My morning I usually drank coffee . Later on I would go to the gym and when I got back home I would break my fast with bone broth or pickle juice then have a protein shake and a half a cucumber and a tea with 2-3 tsp of coconut oil and sometimes butter. Eventually I switched the butter out and started adding lemon juice and cinnamon to try and combat gout inflammation. Depending on the day I’d have 2 eggs but eventually I cut that out as well as I was getting sick of eggs. Then I would have supper and have around 200-300 grams of whatever meat protein me and the family decided and about a cup and a half of broccoli and cauliflower or green beans. Always added a lot of butter and sometimes cheese . I also use both EVOO or coconut oil depending on what I cooked. And I would also have pickles from time to time . I have tried removing pork and it didn’t help I tried removing cheese(was cheddar)and it didn’t help . I have removed coffee and I stopped using turmeric because I heard that could be a problem. I tried eating more chicken as much as I could while on a budget and meal sharing with the family and still this gout hasn’t went away and I still get heartburn. One thing I will note my sleep has also been awful for the past month and I can’t get more then 2-3 hours without waking to pee and usually waking up even sooner then that. I have added omega 3 fish oil and it didn’t help. I have been drinking an electrolyte shake and it only helped with the headache I would sometimes get. My doctor also checked my electrolyte levels and they appeared to be fine . Also I should note I have had hypertension for a while and during the low fat diet it actually dropped to the point I needed to lower my medication but now it’s higher and I had to add medication since starting this diet. I have a condition called otosclerosis, in which, in a nutshell, desensitizes my hearing ability, insomuch that everything sounds either deafened or incredibly soft to me. 110 db sounds like normal speech. I also have some slight nerve damage that further complicates my issue. The problem is that I had to drop out of my college program back 10 years ago because I could not hear students in a classroom setting (I was to be a Spanish teacher) and at the time, I was a poor college student with no insurance (As a foreign language teacher, it is absolutely vital to be able to hear your students. I had zero support from my school and from my community, therefore I had to drop out of the program because I could not afford a doctor, even less, hearing aids. Fast forward 10 years later, my quality of life had suffered immensely because my condition has worsened, the otosclerosis has developed into something worse as I now have osteoarthritis throughout my body, and suffer from gouty arthritis, and kidney disease. I thought I had rheumatoid arthritis (I have medical insurance now, but they still do not cover hearing aids), but I do not. I have a dead-end job that mass quantities of people leave because it is horrible, but I cannot leave as there is nothing else out there for me when I cannot even afford hearing aids.
I don't want to step on anybody's toes here, but the amount of non-dad jokes here in this subreddit really annoys me. First of all, dad jokes CAN be NSFW, it clearly says so in the sub rules. Secondly, it doesn't automatically make it a dad joke if it's from a conversation between you and your child. Most importantly, the jokes that your CHILDREN tell YOU are not dad jokes. The point of a dad joke is that it's so cheesy only a dad who's trying to be funny would make such a joke. That's it. They are stupid plays on words, lame puns and so on. There has to be a clever pun or wordplay for it to be considered a dad joke.
Again, to all the fellow dads, I apologise if I'm sounding too harsh. But I just needed to get it off my chest.

Looking back now, I think I’ve had gout since I was like 22 (27 now). I remember 3-4 occasions from ages 22-25 where I had inexplicable toe/foot/ankle pain. It was never horrible, but enough to make me limp a bit for a few days. I always shrugged it off thinking I hurt myself somehow and just didn’t notice it in the moment.
Then came October of last year. Halloween day. I felt a twinge in my left foot that progressed as the day went on into some pretty substantial pain that had me limping everywhere. However, I didn’t wanna miss taking my kids trick or treating, so I took the “walk it off” approach, thinking that if I get moving, I’ll surely feel better tomorrow.
I think everyone here knows how wrong I was about this “walk it off” theory. What followed in the coming weeks was the most intense pain I’ve ever experienced. Full 10/10. Sharp, shooting pain that would morph into horrible cramping type pain on the fly, and then back again. I couldn’t even comprehend the amount of pain I was in. Sleep wasn’t an option. No position for my foot was comfortable. The ONLY position I could find that allowed me even a tiny bit of comfort, was lightly resting my foot flat on the floor with very little weight on it. Even this position still came with intense dull pain, but it was dull nonetheless. As a work from home single dad, this made it very hard to do what I need to do throughout the day. Luckily I had my mother close by to help a lot during these times.
This went on for about 2 months, pain would be full on 10/10 for 5-7 days, then seem like it would subside for a few days, only to come back. I went to the ER (no insurance at the time) thinking I had some type of infection. They told me I likely have some type of arthritis or bursitis, and to follow up with my GP (which I didn’t have at the time). They gave me some indomethicin (which didn’t seem to do much) and sent me on my way. I didn’t end up making a doctors appointment because I figured it it’s not infection, I’ll just tough it out. And tough it out I did.
I’ll never forget the day I realized I haven’t had foot pain in awhile. That was in December, the flare started in October.
After that horrible experience, I started to research and started reading about gout. All of the symptoms were eerily similar to what I experienced, and the “worst pain in my entire life” theme seemed consistent with my experience as well.
