We have a 10 year old female spayed chihuahua who was diagnosed with CHF, mitral valve disorder, and pulmonary hypertension in Jan. 2020.
She was originally given 6-9 months and put on a regimen of lasix, vetmedin, enalapril, and spironolactone. She went in for an echocardiogram in Jan. 2021 and the vet was astounded that her heart actually seemed to have shrunk and the hole in her mitral valve seems to have repaired itself. No medication changes were made.
Today, the cardiologist checked her again via an echo and said there is no further progression of heart failure and wants to wean her off the furosemide totally. He thinks that the vetmedin is doing the work and that the furosemide isn't really helping and that while she does NOT have any kidney issues, it may cause them in the future.
I am hesitant to change anything. It seems like if its broke don't fix it. I was also under the impression that CHF is a lifelong condition and lasix is a lifelong medication. But she is also going on nearly 2 years from original diagnosis and so we are in unchartered territory.
Please help me out. What should we do?

I take my NCLEX soon and was doing some last minute revision and encountered something I have a question on. So, I know that thiazide diuretics, aside from lowering potassium and sodium, can also increase glucose. I can't seem to find any concrete information on whether the same is also true for loop diuretics. It's making me really anxious because while doing UWorld, you get a lot of questions where clients have diabetes and HF and are given furosemide for their CHF, which is to be expected. I guess I just don't want to confuse myself by thinking that furosemide could also lower glucose levels and then question a physicians orders if he gives a client with diabetes furosemide.
TLDR: Does furosemide increase glucose levels or is that only for thiazide diuretics?
I am a resident and deciding which to become comfortable using in CHF. Most of my attendings use furosemide out of habit. But which is better and why? Also cost difference? Insurance coverage difference? Drop some pharmacy knowledge on me!
The vet prescribed Furosemide as he suspects that Algernon has fluid on his lungs. We started treating him a week ago but haven't seen any improvement. Has anyone else had the same experience and if so, how long was it before you saw any improvement?

I have relatively mild IIH (opening pressure of 20, only very mild visual impairment in one eye that has not worsened) but I want to have something to aid me in keeping my pressure down while I work to lose weight. I can't take diamox or topomax because of a quality of life issue, and being a chef I really can't risk my taste being messed up. I have a neurologist appointment next week and I was thinking about asking to try furosemide/lasix to see if that helps. I've read mixed things about it online and I was wondering if anyone has any experience taking it on its own without diamox or topomax?

My understanding is that Bartter syndrome presents with large elevations of urinary chloride, while chronic furosemide use presents with low urinary chloride. Can I use this to differentiate the two diagnoses?

40/M diagnosed with CHF a couple years ago. I had been in the hospital back then with a heavy duty dose of Lasix to reduce water retention and had a prescription for 20mg furosemide and potassium. After my swelling went down, we had me taking it only as needed and eventually it got removed from my list of prescriptions. Since the beginning of the pandemic, between stress and new depression meds, I gained a bunch of weight that I assumed was due to side effects of the depression meds. I now suspect itβs more to do with water retention as Iβm feeling constantly bloated. I donβt have much furosemide left and Iβm not a fan of taking those gnarly potassium pills with it, so I was wondering if anyone has any recommendations for non-prescription diuretics that work anywhere as well as the prescription kind. I have read a couple reviews of Spring Valley water pills that say they work just as good as furosemide but are more gentle.
So I am a resident and deciding which meds to become "comfortable with". It seems like most docs I know use furosemide because that is what they are comfortable with and so they just continue using it. However, I have heard that torsemide is more consistent in terms of availability and also may have better outcomes. I am wondering if many of you have switched over to using torsemide over furosemide. And do you use it for both acute exacerbations and chronic uses? In acute exacerbations (in patient) what dosages do you start off with for torsemide?
My sweet old pup is in congestive heart failure with a poor prognosis. (As bad as that sounds, Iβm happy to say it after how close I came to losing him this week. Heβs doing ok for now and Iβll take it!) He started on furosemide/lasix this week. He was already wearing a belly band because he was having trouble holding his bladder sometimes. Now heβs wetting them constantly. Iβm worried that heβll get diaper rash while Iβm at work. What can I do? Any tips?
