My cat was diagnosed with CKD last year. We had a close call for a bit but she is doing okay now. On our last visit the vet (who are in a different town so we cannot go there) gave us Renal P powder (https://www.candioli.com/en/products/pets/nephro-urology/renal-p), which I understand acts as a phosphate binder. We have been in total lockdown since mid-March and she is running out and I cannot find a supplier in our town nor any store/supplier who will deliver here. I have also tried to to find Ipakitine/Epakitine but no dice. Is there something else that is more accessible or that I could use instead? At my witsβ end. Thanks for the help.

I just had my labs done yesterday, don't have my results yet but last month I was down to 7% function. I'm pretty sick all the time, the worst is at night . . . I dread the night now. Restless leg syndrome and itching all over. My muscles/nerves feel like they're convulsing and I can't sleep. It's maddening. How does one live like this? Does dialysis help with these symptoms? The itchy skin, muscle spasms, restless leg syndrome? I'm thinking of PD but I have sleeping/insomnia problems as it is so not sure if that will just exacerbate my sleep issues. Also, any advice re treatment/medicine for itchy skin/restless leg syndrome/sleep advice greatly appreciated.
I had my surgery shelf this week; naturally, I decided to write this post instead of actually studying the night before. This advice may not work for everyone, but it's based off of my experiences (and the experiences of others!), and I think it can serve others well. I also think much of it applies to all the rotations in general, not just surgery, and a lot of it is based off of advice I've gotten from other people, but I've also thrown in a good bit of new stuff as well. If people like this I may also write others for other rotations in the future as well. I will withhold shelf advice for now as I have no idea how I did yet.
A little about me: I attend a Top 25 medical school and did my surgery rotation at a large academic hospital that sees a very high daily volume of specialized surgical cases. I'm someone who's struggled with third year from a time-management perspective as a I have a lot of super random hobbies and having to kick those to the side for the year has been quite difficult for me.

1. Be confident and friendly.
This seems obvious, right? Everybody told me this going in, but nobody told me how much it is easier said than done. When you show up and most people ignore you, or they're too busy to pay attention to you, it can be pretty daunting and it's much easier to stand in the corner and not talk. This is not always a bad thing; it's good to know when to keep your mouth shut, so obviously use your best judgement (i.e. if your team is running to a code, probably don't ask people if they've seen the newest Marvel movie). However, you will find that most medical students don't attempt to actually be-friend their residents or attendings, especially on surgery; you will find if you can get people to tell you about themselves, even if they're not initially blatantly nice to you, that they will eventually warm up to you. Many of the residents are over-worked and very stressed and/or may have had poor experiences with other medical students; don't take it personally if you can't win somebody over. ... keep reading on reddit β‘Disclaimer: This is a slight wall of text. I go into a lot of technical stuff here, but at the end I give my explanation as to why we need vertical farms.
So something I have seen dotted around some posts on this sub is a general bias towards low level permaculture over other methods of food production to replace the monstrosity that is modern horizontal monoculture.
While vertical farms can be inefficient in their design, it doesn't necessarily have to be this way. For example, traditionally, vertical farms use grow lights to provide adequate lighting for plants to photosynthesise. This means the whole structure requires relatively large amounts of energy to power. This can come from solar panels, but then you have to produce the solar panels to provide electricity for the farm. There's nothing inherently wrong with producing solar panels. Silicon, the bulk material, is the most common metal on earth, and the dopants that are needed to make the semiconductor more effective, of which aren't exactly the rarest elements either, are used in negligible amounts.
Nevertheless, there's a better, simpler way to approach this, and I think it's really cool.
We already use fibre cables all over the world for things like long distance communication, where electrical signals (through copper) are insufficient. Optical fibres carry no current, and all they're really doing is converging light put into them and "transporting" it to where it needs to go.
Most fibres are made of doped glass, but polymers such as PMMA (Acrylic) work quite well also. Acrylic can actually be pretty sustainable, as it's monomers, although currently not so much, can be derived from renewable sources, and it's quite recyclable (being a polyester, iirc it can be hydrolysed into it's monomers and repolymerised when needed). Also, being amorphous, it's very transparent and thus transmits light very well; this makes it ideal for optical fibres.
The advantage of capturing sunlight and redistributing it throughout a vertical farm is that it requires far less input energy, and the light coming through is also completely natural light.
This solves what I think is the biggest concern when it comes to vertical farms, and the solution isn't too complicated in principle.

Third time is the charm for me! My experience and advice:
I graduated from my internship in May 2021, took about two months off d/t burnout. Started studying in August with only eatrightprep (provided by my internship) and took the exam mid-September, stopped at 125 questions and failed with a 19. My subscores were 13 each (minimum of 15 each needed).
My second attempt I used a 2016 version of the inman, and really focused on memorizing management theories and equations, vitamins minerals, etc (stuff you just have to know, there's no way around it). I rewrote the inman in my own words and reviewed it a few times the week of my exam. I also used pocketprep (highly recommend) and was averaging scores of 60%. I took the exam mid-November, stopped at 145 questions and failed with a 24. My subscore for domain I and II was 15 and for domain III and IV was 14. I was so heartbroken!
This third and final time I used the 2020 Inman and focused on the last two domains which were my weakest points, while also reviewing the first two domains. This time I really pulled everything together and knew the answer to questions without having to look at the choices (This is key!!!). I re-read the inman at least four times and created "worksheets" or mini study guides for major info (foodborne illnesses, lab values, ckd, cardio, etc). This forced me to focus on subjects section by section instead of domain by domain. I found this extremely helpful and much easier to grasp the material. I focused on lab values and how they are influenced by what diseases (more conceptual/situational) and really memorized what foods were good sources of certain vitamins and minerals (the easy sources won't be listed as choices). I have experienced many questions like, "You have a patient with CKD and proteinuria, what foods would you not recommend" or something like that. I continued using pocketprep, now averaging scores of 80-95%. Took the exam January 13th 2022 and stopped at 125 questions, passed with a 29!! Subscores were 17 each!
I was also worried about the new exam update. I did notice more situational questions (and very oddly specific, niche questions I have never heard of or seen anything about on my prior exams) but overall if you know the Inman front to back, the whys, the hows, and study situational questions, you'll be set!

Some background:
I have 2 littermates and they are both CKD and IRIS III. One diagnosed in 2015 (Fluff) the other in 2109. I have tried many, many things and hereβs what works for me. Please note that not all situations are the same, but in my case this is working really well.
Iβm going to be little brief, so ask questions if necessary.
I work closely with my vet and get checkups every 6 months. Both cats are stable and labs are good, but they still have CKD of course. My vet told me that if I can keep them going as they are, she hopes βthey live long enough to die from something other than CKDβ.
What I tried:
First, I read everything on Tanyaβs website. Itβs really A-Z on CKD.
CKD diet was a failure as they just wouldnβt eat anything CKD. I bought everything and tried for months. They are older and stubborn. They eat wet food and treats. No dry food.
I used Mirtrazpine and Pepcid to help increase the appetite and, deal with nashua, which mostly worked. One cat (Fluff) would drink and drink a lot and pee and pee. And puke a few times a day. I had to give SubQ fluids daily and that sucked. She was getting skinny and her fur looked bad. The vet said we had done all we could and asked if Iβd consider euthinasia. For a few days I did.
A friend suggested Rhemannia 6, which I found on Etsy. I was desperate but after reading studies on it, I tried it. I made a tea of it. I fed it to Fluff and it worked. She improved. What I noticed in the next day or so was the incessant drinking had been reduced considerably and she was less dehydrated. I gave her fluids every other day now. Eating was still a challenge but she was measurably better. She stopped puking as much, down to weekly or a few times a month.
Because sheβs a cat, she hated the tea and I ran out of way to give it to her. But not before I found Aminivast. If the Rhemannia was good, this was even better. I no longer had to give SubQ fluids! Plus, it was so much easier to administer. I tried everything I could think of to mask the smell/taste of the tea, but no luck. Manually administering it only worked for so long. Iβd still be using it if I could get them to take it.
I am also using a phosphate binder on treats, but just with Fluff. Zero issues giving this to her and labs seem to show it seems to be working.
What is working for me:

My almost 7 year old beagle lost his 15 month battle with CKD last week and I can't stop replaying the last 15 months in my head at night, wondering if I could have done anything different to prolong his life.
He was diagnosed with kidney failure last September with a creatinine value of 5 mg/dl due to a possible tick infection. The doctor warned that his prognosis wasn't too good at first because we caught it too late. We aggressively treated him with fluids and his creatinine returned to 1.7-1.8 in December. I was ecstatic and he became almost completely asymptomatic.
My nightmare began in January when his creatinine started creeping up again. Truthfully, I stopped giving him fluids from December to January because it didn't seem like he needed it. I thought that fluids are only needed when a dog is drinking a lot of water and I wasn't noticing such a thing.
I started giving him fluids 3x a week again afterwards but his creatinine continued creeping up over the year. I thought that maybe his diet was the problem so I tried adjusting his phosphate binders and phosphorus intake, but to no avail. It was extremely stressful bringing him to blood tests and waiting for the results. It was like watching a ticking timebomb in slow motion. It is fair to say that I was in denial over the situation, thinking that it must have been something else that I have missed, and it can't be him dying from CKD. He was too young to be dying from CKD.
I stopped taking him to the vet for blood tests after September when his creatinine was back at 5 mg/dl because I thought that there was nothing much I could do to change things. The vet also recommended against too frequent tests due to his anemia anyway. It was from November onwards when his appetite very gradually deteriorated along with seizures. I also noticed that giving him fluids seemed to coincide with lethargy so I reduced the frequency to 1-2x a week. The symptoms became to hard to ignore so I took him for another test 2 weeks ago. His creatinine was at 11 mg/dl.
The vet recommended giving my dog fluids everyday and so I did. But his appetite only got progressively worse after that. I brought him to the vet again and although his blood readings improved, he was diagnosed with heart disease, probably arrhythmia, meaning that the fluids could have been hurting him too. His last few days with stomach ulcers were excruciating to say the least. Yes, that was my fault too. Maybe because my insistence on putting him in

We give her 100ml of subcutaneous lactated ringers every other day.
We give her a renal veterinary diet and mix in 200mg of aluminum hydroxide phosphate binder and 500mg of methylcobalamin Vitamin B12.
She still eats and responds to affection.

We want to know what we should do. We've been doing the best we can for her but money is very tight right now. Is there anything we can do at home? Her breathing has become fast and shallow and it's really concerning. She hasn't acted that weird but you can see her diaphragm move pretty significantly while she breathes. Her butt area occasionally twitches. Most vets we've called are not taking appointments today. We're concerned that we won't be able to afford surgery if she needs it.
