Hi all :) I am 26F, recently subscribed to dermatica; I was seeking treatment for mild-moderate active acne as well as preventative ageing. I have been given the Tretinoin 0.015% / Niacinamide 4% / Clindamycin 1% formula.
I have currently using every other night (only used twice so far). PM routine is double cleanse with CeraVe hydrating cleanser, Cerave moisturising lotion, Tretinoin 0.015% / Niacinamide 4% / Clindamycin 1% formula, CeraVe moisturising cream, Vaseline on eye creases and lips.
My question is, am I being overcautious? I have acne-prone skin; only previous retinoid experience is with TO GRE 2% (no issues here at all). Is irritation more likely if step it up to every night?

Hi everyone,
I'm currently a medical student going through my general surgery rotation. A recent case I was involved in has really been eating at me, so much so that I made this account hoping to talk to someone with more experience/expertise. Or at least understand what happened a bit better/hear alternate perspectives. A few weeks ago I was involved in an interesting appendectomy with incidental findings of a tumor that turned out to be a neuroendocrine/carcinoid tumor that was 0.85cm (well-differentiated, typical variety). I'm not sure why I took a special interest in this case, maybe because the patient was so young (only 18), or maybe just because the resident mentioned to me this was a very interesting and extremely rare occurrence. It was their first one too. I'm going through a bit of a hypochondriac phase having recently started my rotations, so given the fact this patient was totally asymptomatic and came in for appendicitis, I spent a few worried nights looking into all the literature with a 'what if it was me' level of detail (procrastinating real studying lol).
What I found was that current 'formal' guidelines seem to find that these T1a appendix carcinoids <1cm essentially don't metastasize and don't require more follow-up. This is currently the course of action my team is taking. However, the patient's pathology report found a number of high-risk features including lymphovascular invasion and serosal involvement. In addition, it was seen by 2 pathologists, the first one who found positive mesoappendix margins, but with the second pathologist who specializes in colorectal specimens finding negative margins. I also found a number of currently published studies on lymphovascular invasion that found tumours >1cm but <2cm with this invasion warrants further right hemicolectomy and lymph node dissection given the increased risk for regional lymph node involvement. As well as a number of studies suggesting that going purely based off size means missing a significant amount of residual disease (granted, I think these studies were referencing >1cm but <2cm tumors, not the ones <1cm).
Anyways, I tried to bring this up to the resident (I was pretty shy about it so maybe that's also the issue), but she was pretty dismissive and said "oh, but the tumor was 0.9cm and those are basically benign". But 0.9cm is pretty close to 1cm, and personally, I feel like those high-risk characteristics are concerning. But I'm just a medical stud

I just got my half sleeve and I really love it since it is my first Japanese style tattoo. However, I have been overly obsessed and extremely cautious about it to the point it becomes annoying. I know I should not care about it much, but I can't just stop myself from doing so. I constantly looking for fading spot even though it just healed for about a week. And I even have plan for touch up in like 10 years from now. I'm really scared that it will fade one day. Have you guys been through this before? Any advice ?

Overcautious: User's speed is halved, 4x super effective moves that check accuracy always miss the user.
There are a lot of bulky slow pokemon crippled by 4x weaknesses like Aggron, Golem, Magcargo etc. This ability would turn an extreme weakness into a massive strength.
Magcargo countering rain with a water immunity would be pretty funny and trick room Aggron would appreciate the switch ins on fighting types.
Who would you give this ability to and to what end?
Im dying to start playing FGO (including story) and im just wondering if I have enough knowledge to start. The only Nasu series I haven't experienced are the Tsukihime franchise, Fate/strange fake, Case Files, Fate/Prototype (LN), Mahoyo, and all EXTRA series past the original PSP game. I'm pretty sure I'm good for like understanding everything but I just want to be sure I'm not too left out of story details.
TL;DR players spending too long being overcautious. What do?
This has been a problem in a few of my games. Basically whenever we are faced with an ominous looking door/object/NPC there's an obligatory 30 minutes of random game actions that basically amount to "we get a surprise round and I use all my buffs before anything happens" and "please don't drop something on us we weren't expecting that we could have prepared for".
Even seemingly innocuous doors/objects/NPCs often get this treatment because someone's encounter sense started tingling.
Personally, I don't mind a little of this as it's usually in character and makes sense for PCs to act this way however it's gotten way past the point where we are spending too much time doing this.
My question is: what kind of things would you suggest I or my DM do to mitigate this, preferably without making a big deal out of it with the other players (I've already spoken to the DM). I've thought a lot about this but I'm yet to think of an easy solution. I am only a player at this point however suggestion's about how to handle it from behind the screen are also appreciated.

EDIT: Feel like I should clarify some things:
-I'm not the DM but suggestions on how a DM would handle this are still helpful.
-We don't have doorway PTSD. We rp our characters avoiding injury but everyone needs 5 mins to say something so it adds up. Plus since it's all we ever really do, if you aren't participating in the pointless overprecautions you aren't playing the game.
-I don't think we metagame. We rp cautious characters and a surprise round is the usual mechanical expression of how it plays out.

So, I'm a DM by necesity. I usually prefer to be a player but because there's no DMs I usually take up the mantle in order to be able to play.
This means I always stick to pre-made adventures as I don't have the creativity to make my own homebrew game, and I usually try to stick to the book if possible. Right now I'm running a game of Dragon's Demand for a relatively new group.
They have a bit of an issue with being overcautious, or overthinking things.
Example (some minor spoilers for chapter 1 of Dragon's demand)
Yesterday they were in a dungeon, and found 7 chests of loot. They spent somewhere around 5-10 minutes positioning themselves in a way that they could be safe if one of them turned out to be a mimic. (One had rolled a high knowledge planes so I didn't consider it metagaming for the party to know what a mimic was).
Later on, they were given the task of driving out a Kobold Den by any means necessary.
They walked towards the entrance and saw some Kobolds standing guard. Instead of approaching to attack, they spent roughly 20 minutes discussing different strategies they should use. One wanted to climb up the wall using their newly purchased pitons, and either drop onto them, or to burn the ropes if they tried to climb after them (the idea was scrapped because then they wouldn't have a way back down and the cleric was a terrible climber).
Eventually, the idea was to have the Wizard, who could speak draconic, along with the Cleric, approach them with no weapons and warn them of an impeding curse that meant they should GTFO.
Now, the problem here is the book specifically says the Kobolds are expecting a retaliation for something earlier and there's no way to diplomacy them. They attack on sight.
So after all their planning and such, I just ruin all their agency with "The Kobolds see you and shout out 'The humans are here! Attack!'"
(once the barbarian finally decided to Leroy Jenkins the whole thing they finished the fight in like 5 minutes).
So yeah, literally my players are the opposite of Murder Hobos.
It makes me feel bad for them because I don't want to railroad them and hate shutting down their interesting ideas. But at the same time, I can't just scrap an entire prepared dungeon because they decide to try and scare the Kobolds instead of going through the dungeon. There's plot points they need to encounter damnit!

Iβve got a problem where the party is over cautious, and Iβm not sure what I can do to help tone it down to a regular amount of caution. At the moment, whenever they travel, the wood elf of the party is always traveling like like 60 ft ahead, talking with animals to make sure the way is clear, and then if she finds anything, returns to the party. On top of that, any time something happens that the party is unsure of, they spend like 10-15 just talking about about things that could happen, even if theyβre not remotely possible.
For example: they encountered this plains area that was filled with animal like statues on the way to a nearby city. They spent the next twenty minutes discussing what could be responsible, if they could fight it, if they were real statues (they were actually petrified, but none of them had succeeded the check to figure that out), and then proceeding like 100 ft at a time asking if they noticed anything else.
Iβve been doing my best to reassure them Iβm not against them as the DM, but I get the feeling that they still feel like Iβm trying to βbeatβ them (theyβre first time players). Is there a good way to reassure them that Iβm not against them?
I understand that everyoneβs got a playstyle and that some players and characters are more cautious than others, but there are a lot of times when the partyβs caution just like stop play for 10+ minutes because theyβre afraid Iβm trying to trick them or they just are too cautious to enter areas (like important quest areas).

As I've been trading regionals the stardust on my main has taken a kicking and is nearly done.
Would I be safe to log in to my main account on my burner and spoof my ass off for one night (whilst obeying cooldown rules) before signing out?

My wife and I are both straight and full-swap.
She's always had anxiety about STDs ever since her backwoods sex education from a middle American highschool. We've been married for 10 years. We have played 4 times in the 9 month since we started. We recently were tested and given a clean bill of health. Our last time playing, we caught something bacterial similar to strep (fever, soar throat, weird taste in our months). We both tested negative for strep and we're all better now, but it has exacerbated her anxieties.
She recently saw ads for PrEP and asked me if I thought we should take it. I'm not sure. I'll do anything to make her comfortable, but on the other hand I'm not convinced that HIV is common enough in the lifestyle to be worth the time, money, and side-effects (nausea is the main one I've seen). I'm worried it will just be a tiger repellent.
Thoughts?
While playing recently I've noticed that a lot of players (especially degrees 40-60 or so) have a tendency to hang outside of the enemy base in neutral flag matches, sitting on re or camping a button, waiting for the FC to die or make some miracle move to score. While the defensive awareness is commendable, it seems counter-intuitive while your team has the flag.
I feel as though it is more effective to apply pressure to the opposing teams defense. That's why the portals are there, if the other team grabs and gets past you it's easy to get back ahead of them.
How do you guys feel about this? Is it better to play D or to apply pressure? If D is better how many people should hang back?
It's also possible that I'm insane and it's a total non-issue.
Juju was in a ton of pain last night (teeth grinding/not eating) and baby gas drops saved his life. Heβs back to normal now, but every gurgle his tummy makes makes me want to give him more drops. Obviously Iβm not giving him more until I need to, but are occasional gurgles normal? Heβs behaving normally and eating and pooping a ton. Thank you for reading.
My Maltese X Russel terrier had been bit by a paralysis tick recently and after the tick was removed I asked my parent whether or not the tick pulled was a paralysis tick or not due to the severity of the toxin. Parent told me not to worry. Yet the symptoms had already shown. But my parent waved me away and said the dog will be fine. It passed away the next day . Why was I ignored . Why is it that being overcautious leads to others ignoring pleads to presume the worst due to their arrogance . If they listened. He would still be alive
