More of an idea than anything but given that the Fate series takes a bunch of cues from the Shin Megami Tensei (and this includes Persona and Devil Survivor) in terms of plot concepts, one idea I had was a Shadow Self (taken particularly from Persona 4). Shadow selves represent the repressed negative thoughts by their real life counterparts, only exaggerated to threatening proportions.
For Mordred, her Shadow Self is made manifest from her desire for her 'father''s approval.
Her Shadow Self behaves like an exaggerated version of her during the Camelot Singularity: basically she's willing to commit genocide and has gone completely off the deep end.
For the design of her transformed version: She resembles basically a MILF version of herself (think Artoria Lancer, in fact she has the same outfit as her), reflecting her strained relationship with mother figures due to her treatment by Morgan. Her leotard has a torn heart symbol, reflecting that she died without knowing what it was like to be loved or cared for. She's also covered in blood and has a pike on her horse having severed heads skewered on it, reflecting the whole war that destroyed Camelot.
Her card would be Wheel of Fortune.

Modred was born in the 6th century in London, England, and lived as the apprentice of a sorcerer. One day, by order of the king he was forced to become a disciple of Merlin, who had recently been replaced by an imposter. However, rather than do so, he attempted to become more powerful than Merlin by going to a tower containing the tome known as the Darkhold. Upon trying to use the Darkhold, it took away his soul and put him into a deep coma. It would only be until the 21st century when he would be revived, imbued with all of the powers of the Darkhold.
Modred attempts to use the powers of the Darkhold for good, but is sometimes unable to resist the thrall of the Darkhold's otherworldly patron, Chthon. Because of this, he bounces between distrusted hero and servant of Chthon.
