I had one gentamicin injection last Friday due to horrible vertigo that was happening 3 to 4 days a week for several months. Iβm happy to report no vertigo so far, but my balance is garbage and I feel perpetually off kilter all day every day since Monday. I have a vestibular rehab appointment this Friday. Does anybody else who has had the shot have any advice or thoughts on how to speed up my recovery? Iβve used up most of my sick time due to the vertigo, so Iβm trying to work through this and itβs incredibly difficult right now.
I was told yesterday that I am likely in Tumarkin Otolithic Crisis. I've had Menieres for over 20 years and have had a near constant flare up for the past three. I was planning on getting the intratympanic Gentamicin shot to help manage symptoms but this new diagnosis has me doubting that decision due to the high failure rate of the shot with this type of crisis. Has anyone had the gentamycin shots while suffering TOC? If you did and it failed, did you have the surgery? Was it successful?
Got my septum pierced a few days ago and my piercer told me to put gentamicin (antibiotic) on it, but after looking it up I saw on reddit that it's a big no-no... thoughts? Should I stop?

My doc now wants to progress to the next stage, with a one-time big dose of gentamicin. According to what Iβve read online, the heroic dose isnβt standard in most of the world. It is however what Iβm offered.
I have some trepidation going forward. On one side, Iβve got very little to lose from taking it. My hearing is already almost completely gone and Iβm basically just surviving between attacks. My balance is whack and I walk with a cane.
In the other hand, what if I get even worse? Can it get worse? Is the big dose so risky I should try for a little longer?
Thereβs also the matter of making a living. My welfare based income is dependent on me βdoing what I can to get betterβ, so denying treatment would mean no income. I have zero savings and losing my welfare would mean losing my home.
Has any of you had the big dose nuclear option? Do any of you have any advice or a perspective to offer?

I had a resurgence of symptoms recently. Went to a new urologist with a new set of labs in hand.
He injected me with a mega dose of gentamicin intramuscular, single dose, stat. Then prescribed oral abx.
Has anyone else been treated like this? This doctor went and extra mile

My mom has been suffering from horrible vertigo episodes multiple times a week since February, she usually ends up in the emergency room and hasnβt been able to work her usual shifts. Iβm her 21 year old daughter who has been helping take care of her during this time.
She recently had a gentamicin injection on August 25th. The injection itself was painful but not as bad as she expected. She has had two vertigo attacks since then, and recently starting 8 days ago has been experiencing the wavy off balance feeling. Mostly she feels like her brain canβt keep up with her eyes, and if she shuts her eyes to rest it makes her feel worse. Overall she is feeling worse after this injection than before.
My mom is feeling very defeated and overwhelmed so I wanted to get some insight.
My question is, how long did your recovery from the injection last? What did you feel like during recovery? Is it better for her to rest during the day or try to do stuff around the house to train her ears and brain to move? And lastly how has the injection worked for you?

Is this administered directly as a shot in the vein or through a drip? My doctor administered as a shot at once and i seem a bit bothered
So Iβm looking into doing a gentamicin injection to hopefully stop/reduce the amount of vertigo episodes. Though are there any side effects? Am I still able to keep balance the same day? Those who had it done what was it like, how was the experience and is it effective?

Has anyone had the Gentamicin shots recently and, if so, how did it go?
My doctor has told me for quite a while that the Gentamicin shot is what I need. In the beginning he wanted to try other stuff like low salt diet and diuretics, but when they didn't work last summer he said to get the shot. He seems very confident.
He says several years ago the protocal was to give several of the shots and just destroy the hell out of the inner ear. Now they do one, maybe two "low dose" shots. So anyone who had it done years ago, or who had a high dose shot, would be dealing with something different.
I was (and am) concerned about balance issues afterward, but he says after about two weeks the other ear will pick up the balancing task.
Yeah, I might lose what hearing I have in that ear, but I don't have much. And I'll be in trouble if the other ear gets it later, but that'll be the case even if I don't get the shot.
I'd really like to hear from someone who has had it done. If that's you, when did you have it done, how many shots, and what was the result? Thanks!
Technically gentamicin relies on peak concentration effect and volume of distribution increases in septic patient. Therefore, loading dose should be given regardless of kidney function.
On the other hand, gent will hang around for 24-48 hours at reasonably high levels in ESRF pts. Therefore, risking otoxicity and vestibulotoxicity.
The literature quoted by different guidelines does not really discuss the volume of distribution issue, or make a distinction between an "infection" vs "sepsis".
