Na v 1.5, the primary voltage‐gated Na + (Na v ) channel in heart, is a major target for class I antiarrhythmic agents. Here we present the cryo‐EM structure of full‐length human Na v 1.5 bound to quinidine, a class Ia antiarrhythmic drug, at 3.3 Å resolution. Quinidine is positioned right beneath the selectivity filter in the pore domain and coordinated by residues from repeats I, III, and IV. Pore blockade by quinidine is achieved through both direct obstruction of the ion permeation path and induced rotation of an invariant Tyr residue that tightens the intracellular gate. Structural comparison with a truncated rat Na v 1.5 in the presence of flecainide, a class Ic agent, reveals distinct binding poses for the two antiarrhythmics within the pore domain. Our work reported here, along with previous studies, reveals the molecular basis for the mechanism of action of class I antiarrhythmic drugs.

What conceivable mechanisms are there by which Nuedexta could/would help a person with OCD/anxiety?
It is an NMDA antagonist and sigma-1 receptor agonist.
The only similarity I can see between it and other drugs for OCD/anxiety are that Luvox is also a sigma-1 receptor agonist and regarded as more preferential toward OCD among SSRIs.
I encountered a prescription today for quinidine sulfate 300mg TID for cramps. I assumed the prescriber meant to order quinine sulfate (which is a whole ‘nother issue) and the patient told me that wasn’t the case. He had tried to get generic Qualaquin approved by Humana and they denied it. Looking at a list of alternatives the MD allegedly decided to go with quinidine(?!).
So, in a dialysis patient with a liver transplant history, does this sound like a terrible idea to everyone else? I saw one n=16 study from 1991 that tried quinidine for cramps, but I’m not convinced. Especially since the total daily dose in that study was 400mg. Is there something I’m missing here?
Please help; in the card they said quinidine inhibits generation of hemozoin , is that correct ? or it's a type

Quinidine is used to inhibit CYP2D6, which converts DXM to Dextrorphan, yielding higher serum/brain DXM concentrations.
What are the ready-available alternatives to Quinidine (supplements/herbs/OTC drugs)? And of these, how much would equate 10mg of Quinidine (the standard dose for 20-30mg DXM)?

I don't want to step on anybody's toes here, but the amount of non-dad jokes here in this subreddit really annoys me. First of all, dad jokes CAN be NSFW, it clearly says so in the sub rules. Secondly, it doesn't automatically make it a dad joke if it's from a conversation between you and your child. Most importantly, the jokes that your CHILDREN tell YOU are not dad jokes. The point of a dad joke is that it's so cheesy only a dad who's trying to be funny would make such a joke. That's it. They are stupid plays on words, lame puns and so on. There has to be a clever pun or wordplay for it to be considered a dad joke.
Again, to all the fellow dads, I apologise if I'm sounding too harsh. But I just needed to get it off my chest.
