I was aboard the USS Greeneville (SSN772) when it collided with the *Ehime Maru*, on Feb 9, 2001.

I can give my POV of that incident, as well as a few others. I can even put you in contact with some other crew members for their POVs as well.

Besides that deadly collision during an EMBT blow, I was also on board for running aground in Saipan about 7 months later just before 9/11, as well as an incident shortly prior to the Ehime Maru where a wave crashed over the top of the sail with the hatch stil open flooding the helm, dive planes, and ballast control panels.

I believe this degree of accessibility could make for an exciting story for YouTube.

What has the USN done to address the training and personnel management shortfalls that lead to the USS McCain and USS Fitzgerald collisions

According to the accident reports poor training and personnel management were two of the key factors that led to the two collisions. According to other articles I've read navigation and watch standing had been reduced to a set of CBTs followed by on board training that was left largely up to the local command.

According to the Fitzgerald accident report the ship was running watch on watch and sleep deprivation was largely a matter of course. Several key crew members that they'd slept less than ten hours in the foregoing 36 hours, some of them a lot less.
