I went into the ER today for an increasing lupus flare that I couldn't handle any more...tight breathing, hives, nausea/vomitting increasing, overall weakness, tingling in my extremities. After labs, chest xray, ekg and monitoring, doc decided with the increase of my inflammation markers, it's probably a lupus flare. What freaked him out the most, my D-dimer came back high 14.80 mg/L (standard 0.19-0.50 mg/L).
They could not do a ct scan with contrast to rule out clotting in my chest and lungs because my creatine is high and my gfr low. The contrast would make my kidneys worse. After consulting with the head of the rheumatology dept at my rheumatologist's clinic, they decided to put me on blood thinners (gave me a dose in the er and sent me home with a prescription to hand in) and increase one of my immunosuppressants (Imuran). He told me to come back to the ER for any little thing, he didn't care what symptom it was, just that I get back in there asap.
I've never been on blood thinners and though I've had my D-dimer increased before, it was never this high. I feel like I am blood clot away from death and that's unnerving. I swear, this stress alone is going to keep me in my flare. Has anyone else experienced this all? I feel in such new territory right now.
I need help to figure this out on which is better
My understanding is as follows:
If you have a low suspicion for PE, d-dimer is not useful because if it comes back positive, it puts you in a difficult situation.
If you have a high suspicion of PE, you immediately start therapeutic clexane and get a CTPA.
When would you actually order a d-dimer?

Went to the er due to chest pain last night where they checked my blood levels and d-dimer test came back in the 1100βs, the did a chest ct to rule out a PE and the ct came back negative. Has anyone else had experiences like this? What can I do to lower my d-dimer levels? When should I get them checked again? I did have omicron about a week and a half ago so theyβre thinking thatβs why they came back elevated but not known for sure.
I happened to have the opportunity to get this blood test and as I hear it... this test tells you how much damage you may have in micro clotting.
Though I got the J&J in late September - my results came back very low. 23. Where normal is under 50.
I had also heard that it was a marker of inflammation as well... and so happy to hear that it was low.
There is very little information out there on the J&J vaccine but at least in my experience that would be what I recommend to anyone over 50.

My motherβs had issues with easy bruising for months, and her WBC (neutrophils, monocytes, and immature granulocytes) have been elevated for several months now.
She went to see a hematologist who ran various tests. Her VW Factor Ag was 274% on 12/14/21 and 252% on 12/30/31. Her VW Factor Activity was 231% on 12/30/21.
On 12/30/21 her d-dimer result was 1,384. On the same date her fibrinogen was 576 mg/dL.
Sheβs been told on a subsequent visit that she has a bleeding disorder, βmild Von Willebrand disease,β and that it explains the easy bruising. He says he does not need to see her again, and no blood tests need to be done to monitor the bruising. I've included photos of the bruising. This happens any time she so much as knocks her arm against a chair. The peeling skin in some photos happened during sleep; she woke up to this.
A family friend who is a NP told her that she does not have VW disease, and actually has a clotting disorder, and to get a second opinion ASAP. Everything I can find online is showing that Von Willebrand disease is related to low VWF, not high, and that a d-dimer that high (itβs been high before: in 2016 she had it tested twice and it was 914 in June, 545 in July) is not indicative of a bleeding disorder.
My mother sees her PCP on February 4th and plans to ask, but was wondering in the meantime if she should consider consulting a second hematologist, or if the Nurse Practioner friend (and the internet) is wrong, or missing some context to suggest her bloodwork is consistent with VW disease.

BTW, Dr. Killian is an Emergency Medicine Specialist and an experienced ER physician in a Canadian hospital who was just terminated because she went public with what she saw the Covid "vaccinationsβ doing. She had nothing to gain and everything to lose by going public.
If I'm shooting a 3 with a guy that has a 99 3ball off a pass from my pg who has hof dimer and floor general, does it just negate my point gaurds badges? Should I run pgs with different badges because everyone has high stats now that are already boosted by my coach.?
I was diagnosed only a few weeks ago with PsA. Currently on Sulphasalazine which has caused quite a few unpleasant side effects (terrible fatigue, muscle aches, tinnitus and a few other milder things). I was in A&E (emergency room) today due to chest pain. Whilst luckily it doesn't seem to be heart related, my D-dimer levels were 900 versus a cut off of 500 for what is considered normal. It's a sign of blood clotting but also can point to inflammation. So I'm now on blood thinners and have to go back in a couple days for more checks. I've had high D-dimer before, went through similar thing and no clotting was detected. So I wonder if could be getting caused by inflammation in the joints.
The ER doctor said it could be raised due to meds I'm on but didn't mention the PsA as being a specific potential cause.
My question is, has anyone been found to have high D-dimer that was explained away as being related to the inflammation of joints and the general effects of PsA? I found a recent study that seems to suggest PsA can increase it but it didn't give a mean elevated figure.
Just wondering whether i need to raise it next time I go to the hospital to avoid possibly needless checks.
I got diagnosed with a bunch of PE in both lungs at the start of December. Had some worrying symptoms last night and the Dr took blood including doing a D Dimer. He said my result was a 'completely normal level' at 0.27. Other symptoms were unrelated and have been dealt with.
So having read the wiki on D dimer tests my understanding is that the number can be low even if you have clots because if your body isn't breaking them down it won't affect your d dimer levels. This tracks because I still feel very out of breath
My question is am I understanding correctly? Is the low D dimer actually probably not good news, and that my body isn't breaking the PEs down? (I have a meeting with relevant drs next week but I'm really thinking about it now).
If so this combination will be lethal alongside floor general.
Hi, I need some help please. I'm 22 male and prior to this was completely healthy and exercised regularly.
I was told by my doctor that my recent blood tests show an elevated D-dimer of 612. My D-dimer has apparently been elevated since around July time (when I had my first ever blood test), but all other blood results are completely normal. I'm not entirely sure what the result was back then.
There's a few things I can say have happened. In May, I developed acid reflux after a week of bingeing before bed. I do feel way better these days and most symptoms are fully resolved (been on medication for a few months) but my upper abdomen feels stingy sometimes still and my throat (below adams apple to the left) also feels inflamed at times, bearing in mind the acid used to hit my throat. But it feels fine at other times. Not sure why this hasn't gone down yet or why it comes and goes when the acid has stopped.
Furthermore, the right side of my upper chest somehow sustained an injury or sprain or something around June time.. I'm not sure at all how it happened but it led to persistent shortness of breath up until now but has improved slightly since. Nothing could be found on any tests to count for this.
I've had X-rays, CT scan, echocardiograms and ultrasounds of my lungs/ribs and they all came back normal as well as functionality. But I'm terrified.. is this indicative of a blood clot or cancer? I haven't actually had the stomach/throat checked from the inside because my doctor said he thought it wasn't necessary but idk anymore. Please help.

This year may it was recorded as LESS THAN 220 (won't show me the exact figure) and now it's 310 - is this something that I should worry about?

I started vomiting Christmas morning, along with the gift of diarrhea. I couldn't keep anything down, so my partner thought I should go to Urgent Care.
There, my heart rate was 120, and the Physician asked if I had any chest pain. I said the previous night, about 8 hours previous, my chest felt tight so I used Albuterol and that helped. I don't have diagnosed asthma, it was my husband's inhaler.
He asked about other symptoms, and I said for the last three days I've been coughing pretty badly when laying down.
He ordered and ECG, and it showed right heart strain. He then ordered a d-dimer and troponin test, and the d-dimer came back positive. They did a chest x-ray and a CT with contrast and both were normal.
They did a Covid test, gave me IV zofran, gave me two bags of fluids, and sent me home with Zofran. He diagnosed me with gastroenteritis. I am vaccinated and have my booster.
Also, my WBC was elevated at 12.7, Neutrophil at 11.2, and my Lymphocytes are low at 0.9.
Next day my vomiting and diarrhea are resolved, but I'm so tired. So unusually tired I'm sleeping all day. I can barely hold up this phone to type. Do you think the fatigue has something to do with my right heart strain? What should I do next? Thank you.
I was just seen at the hospital for chest pains especially when I take deep breaths. I am 25F and have the prothrombin mutation and I am just getting over a mild covid case. VERY mild.
My doctor took my d-dimer test which was in normal range, along with other blood work including INR, prothrombin time, etc. Everything was perfect. Because of this, he did not take a CT scan. Iβve had a few CT scans before and he said he didnβt want to subject me to more radiation if it was not necessary.
I got an X-ray which was normal and I was diagnosed with pleurisy as a result of my recent covid infection.
Since I never got the CTscan, I donβt fully have peace of mind. I struggle with health anxiety, especially regarding my prothrombin gene mutation. I trust my doctor but of course I read stories about people having normal d-dimers and then having clots.
How can I trust that my normal d-dimer means Iβm safe?

34 F, 122 lbs, 5'6, severe anxiety + health anxiety.
I really need some words of wisdom--I am really in a bad place mentally. I suffered multiple panic attacks after receiving my first dose of COVID-19 vaccine on Sept. 8th. This then triggered a state of being depersonalized-derealized and I thought I was losing my mind. I was barely functioning, worried I was going crazy, suffering from schizophrenia, and I had the worst intrusive thoughts ever. Thankfully, through time and exercise, I have been able to calm myself down and get mostly on the other side of that. I still have this now intense fear of death and dying, as well as a constant feelings of impending doom and catastrophic thinking.
I did then decide that I need to get my health and mental health on the right track. I've lost a good 10-15 lbs from lack of appetite and constant running/walking to keep my anxiety somewhat in check, and I now am working with a therapist and getting a lot of normal tests ran--basically a good physical. However, this is where the anxiety has gotten worse.
While running/walking the past month or so, I have noticed random palpitations, sometimes a fluttery chest pain or achy pain bending over or after running but it was always accompanied by gas and stomach upset. I have also experienced a constant lower left abdomen pain, like a side stich pain, but this only shows up when I run or walk fast. I haven't felt any shortness of breath, and I've had off and on back pain but that started after playing tennis this past week. My pulse ox has never dipped below 95 and my BP is fine. I have had a few dizzy spells and body zaps.
During my first Doctor's visit, (note, this was done in my home because I couldn't leave the house) my first EKG came back and it said I had an Incomplete Right Bundle Branch Block, this absolutely terrified me, as I have had many EKGS, echocardiograms and monitors due to fear of heart problems and have never been diagnosed with something like this. I immediately went to Google (I know I should not have but it said possible PE could be a cause of an IRBBB) and I so self ordered a D Dimer through LabCorp, and the D Dimer came back as positive (.90) I immediately broke down in tears and went to the ER.
At the ER, they did another EKG, it came back normal, no IRBBB detected, my Chest X Ray was clear, but my D Dimer was still elevated, now at .82. All my bloodwork came back normal, except slightly low potassium. They wanted to run a CT Scan w
