Hemoglobin Variants Puns

I am a 46 y.o. woman. In July, I did some routine bloodwork and was told that my a1c was 5.8. Fasting glucose was 92. At the time, I weighed 129 pounds and had been eating a lot of ice cream and junk food and not working out enough, but by no means being sedentary. I had a normal BMI and my Dr and I both generally considered me healthy. My Dr and I were both shocked by the bloodwork results. I have never had additional thirst or peeing or any other symptoms.

So for the past three months I have been very careful with my diet. Absolutely no added sugars, carbs only from fruits and veggies and whole wheat bread sometimes in the morning. Kept my carbs to < 100 g/day. I lost 9 pounds in the process. I exercised more and took more intentional walks after eating anything with higher carbs. I thought I was doing everything right.

Well, this morning I got new bloodwork done and the results started coming in tonight. Everything was looking great. Fasting glucose down to 79. Iron levels slightly higher (they are always low), B12 levels slightly higher. And it was taking forever for my a1c results to come in. It came back as 6.2!!!!! There's also a note that a possible hemoglobin variant was identified and that they are suggesting that hemoglobin fractionation should be done.

Can anyone tell me what this might mean? I will call my Dr in the morning, but right now I'm pretty unhappy.

I can't find any information on this online. Does anyone know the naming conventions for Hb variants?

Is that true? What other advantageous mutations exist only in a region of the world?

The Servier Deal: Bittersweet

In early 2021, French pharma giant Servier purchased Agios Pharmaceuticals' oncology division for $1.8B. The newly acquired division includes an FDA-approved drug, Tibsovo, used for the treatment of acute myeloid leukemia and bile duct cancer. Per the deal, Servier is to pay Agios a 5% royalty on all Tibsovo sales in the US. The division also includes a phase II compound, vorasidenib, for the treatment of glioma. Agios Pharmaceuticals will receive a $200M milestone payment when vorasidenib is approved by the FDA as well as a 15% royalty on the total future sales of vorasidenib.

While the Servier deal is most certainly very sweet in terms of cashflow, it leaves Agios Pharmaceuticals in a relatively precarious position. With the oncology business gone, Agios Pharmaceuticals' most promising asset is now the pyruvate kinase-R activator program. The upcoming PDUFA action date is not merely a catalyst, but will most certainly define the direction the company will go. An approval for mitapivat in February 2022 will be crucial to the company as the stakes have never been higher.
