Hi! I recently underwent ultrasound which discovered 3 TiRads-3 nodules on my thyroid. It also noted my tissue was heterogenous and highly vascular. Two nodules are in my left lobe (1.5cm on the mid pole and 1.3cm in the lower lobe) but the largest (>3.5cm) is right on my isthmus pressing into my trachea. FNA was done only on the isthmus nodule and came back benign but did not biopsy the other two despite meeting criteria (my endo was pissed they didnβt but said odds are that they are also benign). The isthmus module is causing obstructive symptoms (feel like a lump is in my throat, choking/gagging sensations, harder to breath laying down). All my lab work was normal, including negative for thyroid peroxide antibodies. My endocrinologist referred me to a surgeon due to the obstructive symptoms and he mentioned I had a choice of either just having my isthmus removed or having my left lobe and the isthmus removed. He said if I kept the lobe, I would need frequent monitoring for a decade due to the moderate risk lesions and have a slightly increased risk of cancer due to the diagnosis of nontoxic multi-nodular goiter. He said if I have the lobe removed, Iβm at high risk of hypothyroidism. My uncle whose a doctor recommended the lobe and the isthmus removed. Iβm curious if folks have been in this boat before and if so, what course of action you underwent? Any questions I should ask my surgeon? Thank you!!

Background:
Outside of a slightly elevated pulse (~75-80 resting versus lower 60s) I have no other symptoms.
Endo is giving me the choice between medication, RAI or thyroidectomy. He sees medication as a temporary solution, with ablation or surgery an eventuality. I'd rather get it over with, and am considering the latter two options. I'm 40, in excellent health, work full time, enjoy a regular exercise routine, and have two small children. I've done due diligence on the surgeon I'd have, should I choose that option, and she is both seasoned and highly recommended. As my endocrinologist explains it, I'm young and healthy enough to tolerate either option, though isolation from my children may be a challenge. And, as I understand it, either way I am looking at levothyroxine treatment. I've read that levothyroxine dosing can sometimes be easier in individuals with no functioning thyroid rather than those with some functioning thyroid remaining, is this true? My gut reaction is to go with surgery though I'd like a sanity check to ensure I'm making a data-driven decision rather than an emotional one. I'm a cancer researcher and am constantly thinking about the dangers of dividing cells, or picking up that second or third mutation that can cause a malignancy -- I think that we as a group tend to be of the "if it's growing and it shouldn't be, cut it out" mentality without appreciating all of the other clinical positives and negatives that go along with such decisions. It's also not clear to me whether a complete ablation is recommended or simply trying to eradicate some of the tissue (is there any truth to it being easier to manage synthetic hormone levels in the absence of any working thyroid tissue?). I also read that RAI carries a minuscule risk of a secondary cancer developing much later down the line. It means something to my psychologically to not have that lurking even in my deepest subconscious. Practically, I don't really care about a scar, and actually a week alone in a cabin in the woods sounds rather nice.

... keep reading on reddit β‘At the end of last year(2014), I started going to a new doctor and went in for a general check up/physical. During the physical the doctor noticed that my thyroid was enlarged, which I wasn't really aware of before, and referred me to the hospital to get some blood labs and an ultra sound.
The blood work came back normal, my TSH level was .71(kinda low?). The ultra sound results showed that thyroid was enlarged and I had 5 nodules in the right lobe(the biggest being 5.2cm) and 6 nodules in the left side(biggest 2.8 cm). They performed a FNA biosopy on the right lobe and fortunately it came back negative. Went to the Endocrinologist and he told me to keep an eye on it and come back in a year for a followup.
Fast forward to last month and I noticed that my thyroid felt and looked a lot bigger and was also sore on top being tight and uncomfortable. The change seemed to happen very quickly, which freaked me so I went ahead and got the blood work and ultra sound done and went back to the endocrinologist. He told me the blood work was fine, but the multi-nodular goiter has grown about 1 cm on both lobes and my two options of treatment are either get the left side biopsied or total thyroidectomy. He seemed to lean towards the TT option so I went and ahead and scheduled an appointment with the surgeon for next Wednesday.
As far as symptoms go, I have that uncomfortable tightness in my neck. My neck looks big but I don't have trouble swallowing or breathing yet. I didn't get the extensive blood tests done but I do have some Hypo symptoms. I'm currently treating depression. I have somewhat low energy. Gained 20 pounds over the last year. However, these symptoms can all be attributed to quitting drinking a year and a half ago too. I also have noticed my hair has been thinning over the past year.
If anyone has any advice, I could use it. The surgery and having to take medication for life is a very scary thing. I can't decide whether this is something I should just go ahead and take care of now or postpone the surgery and just get the biopsy and followups. Thanks.
I don't want to step on anybody's toes here, but the amount of non-dad jokes here in this subreddit really annoys me. First of all, dad jokes CAN be NSFW, it clearly says so in the sub rules. Secondly, it doesn't automatically make it a dad joke if it's from a conversation between you and your child. Most importantly, the jokes that your CHILDREN tell YOU are not dad jokes. The point of a dad joke is that it's so cheesy only a dad who's trying to be funny would make such a joke. That's it. They are stupid plays on words, lame puns and so on. There has to be a clever pun or wordplay for it to be considered a dad joke.

I feel like I must seem like such a weirdo because I am hopping from disease to disease board trying to find answers.
I was told I have a multi nodular goiter in April. I was sent to UCSF to have a FNA. They did a really detailed ultrasound and the radiologist said no need for FNA. He said my bigger nodule (2 cm) was fluid filled. He thought the other 'nodules' were scar tissue.
After my second pregnancy I have started having steadily worse health issues and a lump appeared on my neck on the right side. It is still there but I have gained so much weight I can't see it as easily.
I have gained a lot of weight. 70 lb over 4-5 years. Joint pain. PVCs. High Blood pressure, stretch marks, swollen stomach.
I grew up in Ireland in the 80s which a doctor told me was relevant for iodine deficiency.
All of my blood work is normal. Over the years I have had numerous positive ANA tests but the specific tests for lupus etc are negative.
Normal T4. T3, TSH, Thyroid antibodies. Normal Dex suppression test.
My questions are:
Can you have thyroiditis with normal blood work?
Can you have both hypo and hyper thyroid symptoms with thyroiditis?
Do your worst!
