Pediatric urology drfectd

I'm curious about congenital anatomical abnormalities that would cause a 1 year old uncircumcised male to have recurrent UTI's with various bacteria (e-colli, psnuedomoni). Would this usually be a valvular issue causing reflux, or something else? I've had several adult male patients with recurrent UTI's but these seem to be related to prostate issues. I assume in peds this isn't the same issue?

##Hello friends!

Apologies for being a few days late to post the latest addition to our ERAS megathread series. Hope you all had a great holiday season! We are now in month 4 of interview season! This is the month 4 megathread, please concentrate all interview, ERAS, rank list, etc. related posts to this thread. Feel free to ask all your questions and share your experiences here.

As we start the new year, lets reflect on far we've come! From hunting down LOR writers, perfecting personal statements, to refreshing our email inboxes every 2 minutes, to creating the best virtual interview set up, we've been through a lot. Remember, the interview season goes all the way until March, so keep refreshing those email inboxes! You never know when a surprise late season invite might come through.

If you have an excess amount of interviews at programs you have no interest in attending, it is encouraged to drop them to facilitate a trickle of interviews down to those who don't have as many interviews.

We're in the final stretch of the cycle now. I'm anticipating there will be a lot of questions regarding rank order lists, second looks, and letters of intent.

Getting to the end of third year and I'm more confused than ever about my specialty! (gen surg vs. urology vs. EM vs. ???)

I am getting towards the end of 3rd year and feel like I should be ruling things out but the only rotation I explicitly did not like was psych. And I have two more rotations left and what if I end up enjoying those too! I feel like I have so many options but I am struggling to find the right one for me.

- Gen Surg:

Pros: I feel like I really gravitate towards surgery because I love the team environment, working with my hands, spending time in that moment focusing on this one patient, and I feel excited looking into someone's body. General surgery has a lot of breadth and a lot of different types of procedures, which I enjoyed. There's also a certain joy I have in touching people's guts and I have fun with the bread-and-butter cases. The residents at my home program were nice even so it gives me hope about the future of surgeon personalities. General surgeons also seemed to be some of the most knowledgeable doctors I met- they did surgery but also managed people's medical conditions instead of punting to a medicine consult.

Cons: Lifestyle obviously. When I talk to non-general surgeons and say I'm interested in gen surg, they wince and ask why. I have a husband and want kids someday so this worries me. I had variable experiences with attendings, but they still seemed to work quite a bit. Being more a generalist sounds good to me but I know that's not really where the future of gen surg is going (everyone seems to do fellowship).

- Urology

Pros: The surgery pros like gen surg but the attendings I met in urology seemed happier. It seemed that they had more of an opportunity to have a life outside the hospital. Renal, pediatric, and female reconstructive surgeries were super cool. I also enjoyed the small kidney stones cases and TURPs. There seems less pressure to do a fellowship, so you can get to being an attending faster.

Cons: With there being more male patients than female ones, I was asked to not be a part of few consultations because I am a woman. I also don’t mind the bread and butter (BPH, difficult Foleys) but I’m not passionate about it. It's stupid but there is something inside of me that hesitates on being a "dick doctor" as many urologists I met put it. On a practical level, I don’t have any urology-specific research so I worry about my chances but my home program PD said I could still have a good shot at matching if I get started soon.

What’s the highest # of residencies anybody has ever done?

I’m thinking of starting with a nEuroSurgery residency and then once finishing doing a pediatric-cardiothoracic residency, taking a year off to research bodily-discharge pathology and then doing an osteopathic-geriatric-urology fellowship. ThoUghtS?

Late interest in uro, how to move forward?

3rd year here that recently did a urology elective, which I initially only chose for the chill hours, and ended up liking it way more than any other rotation I've ever done. I never had any sort of interest in a competitive specialty before because I was set on FM so my resume is pretty lackluster. But now I'm sure urology is what I want to do if I can pull off matching into it.

Here's what I'm currently working with: Step 1: 241. Preclinical grades were all average. Clinicals have been a mix of honors, high pass, and one pass. I have 3 case presentations in unrelated fields (pediatrics and IM). Really don't have any special ecs or anything (spent too much time playing Dark Souls during preclinical). I have a home urology program but there's only 2 spots and I know there's at least 5 people in my class that have been working with them for a while.

Do I have a shot at urology? Or is it best to just move on. Is there any way to build a competitive resume before the next cycle? I'm not opposed to doing a research year but if I go through that and can't match... I don't even want to think about it. Not to mention I have no idea what'll happen with my Step 1 score if I apply in a year later because everyone from now on will be P/F.
