Deep rabbit hole. I started by thinking about some basic math for the federation. How much antimatter fuel does the federation use in a year? where does it get it? How does having that much power available affect the "economics of the 24th century"?Here are some back of the envelope calculations that I've come up with.
I found a link here, that basically infers that antimatter is produced as a result of fusion reactions, at an energy loss of 24% (as it's basically stored fuel, you have to spend more energy than you get out of the fuel)

we know that the starship enterprise burns through around 1900 Kilograms of antimatter fuel/day (about 4000 pounds).
Here, we read that the staple of the federation fleet is probably the excelsior class (a similar size to the enterprise) and that depending on estimates starfleet runs around 20k-30k. I'm going to take the lower number there. I'm going to assume that about 1/2 of starfleet is either an galaxy class or excelsior class and runs similar power requirements, and that the other half, runs about half of that, as we can assume that on average they are smaller ships with lower power requirements.

So, that gives us about 10K ships that run 1900 kilos of antimatter a day, and 10K ships that run about 950 Kilos/day. That runs the federation fleet at around 28,500,000 Kilos of antimatter per day. Since we can infer that there is an energy loss of 24% from a fusion reaction to create antimatter fuel, that means that the federation needs to run about 37,500,000 kilos of hydrogen fuel through fusion reactions, per day to fuel starfleet (and we are neglecting planetside power requirements so far)
The fuel for fusion? Deuterium. Deuterium, has a natural abundance of 1/6400 in earth's oceans. In season 5's Ethics, we see Worf with a debilitating back injury, and the proposed technology used to fix him is something that logically already exists. A protein synthesizer/replicator capable of replicating a genetic code, growing a new spine and transplanting it.
Medical replicators should be capable generating a DNA strand, it is self replicating, you just need to build 1, and beaming technology already dematerializes spinal columns on the daily with no ill effect.
We know genetic enhancments are banned in the Federated human worlds, possibly considered so unacceptable that until the 24th century it never occured to anyone to do what this new doctor is proposing, even though it should be unbelievably doable for the days technology. Possibly it gets too close to genetic engineering? Opening the door to novel code being used for over powered organs?

Also, it would be a Constitutional Monarchy with Otto Von Habsburg as Monarch and would consist of Austria, Hungary, and Czechoslovakia. South Tyrol would be ceded to the Federation and all people groups would get equal say and rights within the nation.

I will discuss in this post the invasive, numerous, and near body-horror-like modifications that the Orbital Guard (also known as the O-Guard) must receive in order to thrive and survive in the field. I appreciate any feedback, suggestions, and any potential loop holes I may have missed.
First thing I'd like to point out is that the body mods are meant to be extremely invasive to the degree that they might not be considered humans. This is supposed to be a part of the story to show the sacrifices that the Federation will make for people in order to grant security to their few home systems.
Second thing: these people work on small-medium sized vessels that are designed to handle extremely intense maneuvers, intense and long burns, along with extreme endurance patrols out in the out rim of any system. Primary function is as the 1st line of defense they must harass and disrupt hostile forces as much as possible until a proper response force can engage the hostiles. Now that you understand their role and hopefully the intense tasks the bodies of the crew must handle lets start on the body modifications.
Both legs and arms are replaced with sturdy titanium-alloy limbs that can all function as hands if need by for manipulating any system or device they need. The limbs will also be capable of withstanding much more punishment than any organic/human limbs.
A computer port will be installed near or around the brain(or stem) so that the crew may interface directly with computers allowing for the most efficient use of a vessels resources. The crews have come to refer to this as their "third eye".
Many of the muscles have been replaced with synthetic tissue one that does not degrade from long term exposure to low-zero G's. Unfortunately this also prevents many of their muscles from strengthening through exercise. Muscles like the heart and lungs simply have small machines that stimulate the muscles to prevent degradation.

A problem with this surgery is that they will forever be dependent on liquid breathing, though this will only complicate the lives of those who survive their service.
